What Does My Serum Analysis Indicate About FSH Levels?

Question: Hi Doctor,

Last week I have done serum analysis test and blood test. No Sperm(azoospermia) detected in report. Detail od report is as follows:

FSH : 26.01 (was 25.22 in October 2013)
LH : 9.08(in October 2013)
Testosterone : 563.91 (was 453.91 in October 2013)
T3 : 122.20
T4 : 7.04
TSH : 1.93
Prolactine : 70.73

What this report says exactly? My local gynac is asking me to do biopsy, but another known doctor is saying to avoid biopsy and directly go for tese-icsi so that I will avoid loss of sperms in biopsy. Do you really think that chances of testicular failure? Confused, please help.

Hello Sir, thanks for trusting health care magic.
Your report is clearly suggestive of testicular failure because of high FSH and azoospermia.
Biopsy is the ideal test you need to document if you have sperms present in testis or there are none.
In case you have sperms then the treatment is ICSI or TESA and for that matter just one biopsy won't damage much number of sperms.
Sperms if present will be in multiples of hundred and every cycle of IVF will be needing fresh biopsy from testis to get sperms and they will keep on forming thereafter.
BUT in case no sperms are found on testicular biopsy then no need to think in line of IVF (which is a costly procedure indeed) and a donor semen in utero insemination (IUI) will be done.

Hi Doctor,

Thank you for your information. I heard there are increases chances of finding sperms using TESE or Micro-TESE. Can you please tell me that how much biopsy, tese and micro-tese treatments will cost? Also, can you suggest some good Clinics in Mumbai where I can check these treatments please..

testicular biopsy gives better results

Hi sir, the chances of sperm retrieval are maximum in testicular biopsy only especially when we want to know if spermatogenesis is occuring or not.
Epididymal aspiration is much less invasive and if sperms are obtained patient is saved from the trauma of testicular biopsy everytime in every cycle of IVF (In case multiple cycles are required)
Cost of testicular biopsy varies from anywhere between 10-15 thousand rupees plus the medicine cost.
Mumbai is the hub of infertility centres and you can get ample information on internet and choose according to your convenience of approach and transport.
And for that matter testicular biopsy is a simple procedure and can be easily done and assessed by all clinics.

Hi Doc,

One last question, I have normal Testosterone but have very high FSH and LH. I read somewhere that if you have normal testosterone then very less chances of having testicular failure. Also my gynac told me that I have small testicles. What you suggest for this?

small testis is not in favor of normal sperms

Hello sir, there are many cases where testosterone is normal but still azoospermia and especially in your case because you have small testis.
Best modality of diagnosis will be by testicular biopsy.
But if normal spermatogenesis is seen IVF will be the option for having your biological child.
