Posted on Mon, 18 Jan 2016
Question: I am 31/M and my semen analysis report on 5th october suggested that, I have total sperm count of 25 million millions/ml and under motility it says: active motility: 20%, sluggish: 50% and non motile: 30%.
My wifes gynaecologist suggested two Fertisure-M tablets per day and since then I am using this medicine. But now slowly my interest in love making is diminishing and my errections have not been good.

Couple of years back I had Urinary incontinence problem, then it was diagnosed that my Prostate gland is enlarged but after medicines it came back to normal size and problem was fine, but recently I am experiencing this problem also since a week.
Earlier I was fine. I also have cervical spndylosis problem of neck and my doctor has given me zincovit tablet (one per day), vitamin D powder and amitryptyline ( one per day in evening) and I am taking these medicines since 3 months. I do walking every day and my diet is also pretty good. I dont know what is the problem? are my medicines causing any problem. please suggest what needs to be done.

Normal semen analysis

our semen analysis is absolutely normal. Please stop medicines given by gynecoloist. They are not needed for you.

Amitryptiline is known to cause ED. Please ask your spondulosis doctor if this can be changed or modified.

Dear Dr,
Thanks a lot for the reply.
I will stop amitryptyline and fertisure-m medications.
But what is the reason, that I am experiencing urine incontinence problem ( urine leakage), it is affecting me psychologically.Although its just a few drops, I am not able to concentrate on my job properly.
Is it also because of the medication I am having?
Please advise regarding this problem.

Just stoping amitryptyline will solve the problem of erectile dysfunction or is their anything else which needs to be done? Please advise.
Thank you.

Dear Dr,
I have uploaded few of my tests such as a blood test and KUBP ultrasound scan which were taken in the winter of 2014. I had gone for these tests on my own and then I didnt had much problem later.

I had started having urinary incontinence problem in winter of 2013. Then I had visited a doctor who prescribed ultrasound (kubp), some blood tests and urine test. All the tests were fine except there was some enlargement of my prostate gland. But with medicines which he prescribed me, it came back to normal size. He also prescribed me for some tests to check for prostate cancer, but all the tests were fine (negative). And after winter of 2013 the problem also subsided.

And now since some ten days I am experiencing this problem of urinary incontinence. Its not a lot of leakage but just a few drops, that too it doesn't happen all the time. It occurs mostly when I am sitting on my table and studying some thing. When I am doing work on my desk I could feel uneasy that something is happening and I am not able to concentrate on my work and it is disturbing my psychologically. Please advise what needs to be done.
Thank you.

Do Kegel's exercise

Detailed Answer:
Hi,

I dont see any big problem in reports. There is nothing to worry about.

All you need is Kegel's exercise to improve strength of perineal muscles.

Please read about kegel's exercise in google and let me know if you find difficulty.

My physiotherapist has already prescribed few exercises for my back and neck.
For core strengthening of back he has prescribed doing exercises by engaging my core. I have seen regarding kegel exercises in google as well as on youtube. I think kegel exercise is similar to the exercises prescribed by my physio.

But I have a doubt regarding these exercises, in locating the pelvic floor muscles which needs to be contracted. In some videos Some sources say, while urinating, the muscle which one uses to stop it, ie your pelvic floor muscles and in other sources say that we have to push our belly button towards the spine. In both of these methods which is the one, shall I follow for kegel?
Please advise.
Thank you.
