What Does My Lab Test Report Indicate?

Question: Had a miscarriage. Scanning says:
Findings: Urinary bladder is well distended. Wall thickness is normal. No evidence of calculi, mass or mural lesion.
Uterus: is anteverted and normal in size, measure 6.9 x 4.5 x 3.2 cms. Myometrial echoes is normal. Endometrium is thickened with irregularity of lining. Endometrium measure 13.2mm.
Both ovaries are normal in size and echopattern.
Right Ovary: measures 2.2 x 1.8cms
Left Ovary: measure 2.3 x 2.2 cms
Both adnexa: Normal. No mass is seen.
There is no ascites.
Impression: Thickened Endometrium - likely retained products of conception.

My doctor is asking me to get D&C done. Wanted to know is it really needed or medicines alone can help?

1. Your ultrasound report shows that after the miscarriage, the contents of the uterus did not empty completely. It means some parts of the small baby are still inside. D and C is required to remove this otherwise you might get an infection.

2. Uterus infection is difficult to treat and there can be septic that is risky to your life. Therefore the doctor is right in suggesting D and C for you.

3. Medicines like misoprostol are available to clear uterine contents from early miscarriage. It is possible that your pregnancy was slightly advanced at the time of miscarriage or medicines have been given and still it has not cleared. Therefore doing D and C might have been suggested by your doctor. Please discuss this matter with the gynecologist who is treating you.

I had taken Regestrone from 17th july till 4th of this month because of travel and I did not know I had conceived.
No medicine was given to clear and she just asked to get D&C done. how do I know how advanced was my pregnancy? I am really worried about D & C and wanted to avoid it.

Thanks for writing back with an update.

1.Regesterone is Norethisterone which is a medicine given for birth control, endometriosis, excessive bleeding during periods and as a hormone replacement in those who require it.

2. Probably you have been taking it for birth control and I guess that it might be contraceptive failure which caused the pregnancy. The age of pregnancy is known from the ultrasound scan which might have been done before the miscarriage. If no earlier ultrasound scan is done before the miscarriage then the age is calculated from last onset of periods.

3. I suggest you discuss medical evacuation of retained products of conception using misoprostol with your doctor and explain your unwillingness to do D and C without trying medicine treatment. Please note that misoprostol treatment is strictly under medical supervision and it is left to your doctor to decide on that and after keeping in mind your symptoms and reviewing your case in detail clinically.

Doing another ultrasound scan is important after taking medicine or doing D and C to know if complete contents have been removed and uterus is empty.
