What Does My Blood Test Report Indicate?

Question: Dear sir my son is 3 years old, we have got his blood test done recently the report says RBCs show moderate anisopoikilocytosis with moderate hypochromia. Microcytes, target cells and pencil thon elliptocytes are present". We got this test done because my son had tendency to eat soil and clay. His haemoglobin count is 10.0g/dl serum iron is24.00 ug/dl

Welcome to the HCM

I have gone through your question and the report attached. I completely understand your concerns. Your child is suffering from iron deficiency anemia which is a common reason for children who suffer from Pica I.e. craving for non edible things like clay, mud , etc.

The reports also suggests the same. There is a discrepancy in his RBC count and his hemoglobin and other parameters ( MCH, MCHC, MCV,PCV) which may also occur in thalassemia trait.

So, I would recommend you to start your child on iron supplement such as Ferronia XT ( 6mg/kg/day of elemental iron) for next 2 months and repeat his complete blood count there after. If the parameters do not normalize, then go for HPLC test to rule out thalassemia trait.

Also, give him albendazole (400 mg) once at bedtime and repeat the same after 15 days for deworming. This is important because children may suffer from intestinal worm infestation due to Pica.

Thanks for your advice, he recently had a upset stomach, which is better now. So shall we start the iron dose immediately or wait for some more days.?

Also I didn't understand the dosage told by you ( 6 mg/kg/day)

Detailed Answer:
Hi
You should wait for a week after stomach infection before starting iron supplement.

6 mg /kg/day means that if your child is 10 kg...
Then give him 6 x 10 i.e. 60 mg of elemental iron daily. Since ferronia XT has 30 mg iron in 5 ml...so give 5 ml twice daily.

So, calculate it as per your child's weight.
