What Does My USG Scan Report Indicate?

Question: My USG shows liver is mildly enlarged shows mild diffuse increased parenchymal echopattern.No focal mass lesion Is noted.IHBR are not dilated. Portal vein is normal.
Accessory splendid of size 13.3×10.5 mm is noted at splenic hilum.
Gall bladder is partially contracted. Lumen is echo free. Wall s are normal.
Please tell me about my situation. Is my liver is going down failure.
Now after 1 month of usg my abdomen is slight more swelled and I have a mild gas type pain in both right and left side of upper central abdomen. My liver function test and cbc, urine test is normal. My tsh level is 17.14. Pls tell me there is anything serious or life threatening.

I am also suffering from psoriasis for 18 years . A dermatologist prescribed me two tablets in a week. Can I take the medicine in such condition liver. And what does mean of diffuse increase parenchymal echopattern. My total health is thin only my abdomen is fatty. Is it possible due to fatty liver.

Thanks for writing back. Diffuse increase parenchymal echopattern refers to fatty deposition in liver but that does not equate to fatty liver. Fatty depositions in liver can occur due to a number of causes. The first question I would need to ask is whether you are an alcoholic. If not, I would need to ask you whether you are a diabetic. If yes, the blood sugar levels need to be controlled with regular medication. If you are not diabetic, there is an entity called Non Alcoholic Hepato Steatosis (NASH) [hepato steatosis = fatty depositions in liver] which needs no intervention.

Regarding the medication for psoriasis, could you please provide the name?

Yes doctor.
My physician provide me for psoriasis tab Methotrexate 7.5 mg and Folvite 5mg. I am not an alcoholic nor diabetic. But before 1 year I have regularly taken coca cola, Pepsi as soft drink in summer.
Further. Pls tell me is it possible that my abdomen is only swelled due to fat in liver as I am very thin person with all of my body except abdomen.

Thanks for writing back. I would insist you to take methotrexate as directed. It has to be taken with folic acid. Good to know that you are not alcoholic or diabetic. In such a case you do not need to bother about the fatty deposits in liver.

Your abdomen is not swollen due to fat deposits in liver. In fact it is unrelated. You have central obesity (which can be present in thin persons). You need a proper exercise regimen.

I would insist you to restrict carbohydrate and fat intake but maintain a modest protein intake. Do the following exercises daily (google out for technique). No. Per day with number to increase per day (in brackets) provided:
Push-up - 10 (2-5)
Crunches - 20 (5)
Planks 20 (10)
Squats - 20 (5)
Triceps dig - 5 (2)

I have a mild pain in left side of abdomen. Is this may be for central obesity. Or it's due to accessory spleen. Is an accessory spleen is harmful because I have an accessory spleen.U must listen in the reports I have attached earlier.

I always feel gas in my abdomen. Is this possible due to obesity. What can I do for relive from gas symptoms. Whenever I feel gas I feel my abdomen is more tendered.

Likely to be due to slowing of the gastrointestinal movements

Detailed Answer:
Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation. The abdomen therefore feels distended and the associated gastritis could make your abdomen feel tender.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep (fluoxetine) 20 mg tablet (long term anxiolytic and a prescription drug) once daily for at least 2 months (it may take time for the action to set in). Discuss about this with your treating physician.
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Stress reduction has a very important role to play. Yoga and meditation should help.
* Obesity is only indirectly related. It has no direct role to play.

A last and final question.
In my condition is it possible to sweat more and more. Because I am doing such. Is there is anything serious because I have an afebrile on elbow and 20 or 25 liver spot on whole body. Sometimes I have a mild fever also. No headache no vomiting.
