Question: Hi! I am having a marked increase in sciatic type pain. The pain has increased so much that I am taking oxymorphone & was given dilaudid in he ED. My neurosurgeon looked at the MRI, which showed a large central herniation with moderate spinal stenosis. He thought it was unchanged from one done a few months ago. Even with taking the oxymorphone, it is still so painful I can barely walk with crutches. My hand surgeon husband told me I need to rest, but from what I've read, you're supposed to remain active. What is the answer? If it is rest what kind , how much, & how long? Also I have atrial tach, am on Tikosyn ,& have had 2 cardiac ablations without success. Do I have any alternative besides another ablation attempt? I am 58. Thanks so much for your time!

Hi there.

Having large verbal disc protrusion can press on nerve roots of both the sides causing severe sciatica symptoms.

You cannot keep taking medication all your life. You may require surgery especially if you have a spinal canal stenosis. Avoid bending forwards or lifting heavy weights. Apply hot water bottle to the painful regions. You need to do brisk walking exercise to keep your BMI in the normal range and avoid obesity which can make the problem worse. No need for bed rest. You can move about, but avoid sitting or standing for long time.

Regarding the Supraventricular tachycardia, you can discuss with your Cardiologist about an implantable pacemaker which will help prevent the episodes. But ablation is the only cure apart from maintenance with medication.

Kindly do tell me the canal diameters as mentioned in your MRI Report.
