Question: Alt is 1268/ Ast is 1381/ get is 143/ creatinine is 75/ urea is3.4/ cholesterol is 5.62/ alkaline phosphatase is 157/bilirubin total is 144/ bilirubin direct is 104/ chloride is 102/ sodium 135/ platelets 422/ row is 17.1 WBC is 9.6 I am female age 56 am jaundice eyes yellow / itchy skin / dark urine. Doctors say problem with liver but don't know what exactly iam thinking primary biliary disease

Brief Answer: Conjugated Hyperbilirubinemia, work up needed Detailed Answer: Thank you for asking! Your liver function tests are markedly deranged and conjugated hyperbilirubinemia needs to be worked up for. One thing is for sure liver is doing its job to convert the heme products to bilirubin till conjugation so the issue lies in the clearance from liver onwards. Obstruction at some point is suspected from biliary tract till duodenal sphincter.The itching and direct bilirubin sky high indicates that. Now geriatric age of 56 with no previous history of such presentation or liver disease, i suggest you seek a hepatobiliary consultants for a bit further work up like MRCP, ERCP. Transhepatic cholangio pancreatico grams THC, CT Abdomen, Complete pancreatic profile with CA 19-9 and HIDA scans and ultrasounds to sort out the etiology. Its too early to diagnose the cause for this conjugated hyperbilirubinemia. A long list of medicines and environmental exposures to many obstructive cholangiopathy and malignancies of biliary tract can explain it. We need to narrow it down and connect the dots here further.
