What Does My HCG Report Indicate?

Question: HCG Report
1) Retrograde filling of the left fallopian tube with free intraperitoneal spillage of contrast
2) Partial visualization of the right fallopian tube. During the course of the examination, there was preferential spillage on the left side which eventually obscured visualization of the right fallopian tube.
Patency of the right fallopian tube cannot be really assess on this examination.

I am 40 years old and want to have child through natural means (without IVF & IUF). Does this HCG result implies one of my fallopian tube is blocked and I have 50% chance of getting pregnant compared to regular woman.I have attached the original HCG report too.

Since your HSG report is not very conclusive I would advise you to undergo Diagnostic laparoscopy. It will very clearly delineate the anatomy of the fallopian tubes. Since the right side of the fallopian tube patency is not very well defined,diagnostic lap is the next procedure. Will it be ok to ask that for how long are you trying to conceive? I got married at the age of 40 and as such we had peer pressure to get child soon.
We tried for 5 months and at the end of 5th month we went for IVF treatment on basis of doctor’s referral.
A famed IVF clinic in New York City sold us the whole process like a dream & charged us $26,000 for a single cycle of IVF.
The whole IVF process lasted 2 months which means at the end of 7th month of my marriage I was told that the IVF Cycle was failed.
The same IVF doctor with degrees from XXXXXXX medical school who had promised us miracles now told us to buy 3 cycle of IVF worth at least $50,000 saying it is pretty normal for IVF to fail 3 times.
We realized that this whole IVF or IUF is a big scam and it is better to run when we still had time.
As referred by you we do plan to go for Diagnostic laparoscopy but I have a question if right side of my fallopian tube is not found to be patent do I still have 50% chance of getting pregnant compared to regular woman.
Answer to this question is very important to me because I have decided not to go for IVF/IUF process & try to have a baby the natural way

I understand your despair about the treatment outcome which is natural and common.
But frankly speaking your doctor has told you the exact situation and did counseling as per protocol.
IVF treatment is indeed a costly affair with very less success at your age but the percentage of success is Still high compared to natural methods.
But as you don't want IVF and want to try naturally then I would like to increase the chances of success and target your difficult areas so as to have maximum benefit with minimum input.
At first please tell me the report of AMH (anti mullerian hormone) levels and then get a diagnostic laparoscopy done to know the actual status of tubes and ovaries.
Patency and tubo-ovarian relationship both are very important in natural conception.
In case your one tube still comes blocked then chances of conception decrease by 40-45% as usually egg forms alternately in both the ovaries.
In some fortunate cases egg gets transported to the patent tube from the ovary on the side of blocked tube so it is a matter of luck too.
Just get the investigations done and then we can discuss further management.
I basically practice infertility and deal with at least 15-20 infertility patients daily so in case you have any other questions please feel free to ask.

My AMH (anti mullerian hormone) levels came out to be 0.21 ng/ml which is low as I understand.
But my ultrasound revealed 3 follicles in left ovary & 2 follicles in right ovary – a total of 5 ready to go.
As advised I will get Diagnostic laparoscopy done so that we can discuss further management

In case your follicle count is good then chances of spontaneous pregnancy are still there with minimal assistance but don't waste much of time and take all the decisions as early as possible.
