What Does Low TSH Value Indicate?

Question: Hi. I just had blood work done. All was normal except my TSH for thyroid test. Never had a low reading, mine came in at .269. I read .3 is the bottom end. Doc wants me to come in for discussions says possible ultrasound and biopsy. Any thoughts, advice or input appreciated. It has me worried. I am a 45 yr old male.

Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
A low TSH indicates Hyperthyroidism. We need to find out the cause.
Before i give you my advice on diagnostic workup and treatment, i would like to know a few details:
1. When did you get your TSH done before this (which was normal)
2. Have you lost any weight?
3. Do you have Palpitations?
4. What is your Resting Pulse Rate? (Count it for a minute by yourself and tell me)
5. Do you feel anxious?
6. Any constipation or Loose stools?
7. Any Recent illness like fever , Viral Infection.
8. What are the T3 and T4 Levels?
9. Have you any swelling or Lump in throat?

I am asking all these questions to see if you have any signs or symptoms of Hyperthyroidism, Thyroiditis.

Last set of comprehensive blood work was XXXXXXX 2014 and all was normal minus a slight lower TH test. before that last summer all was normal and within range.

I have lost about 6 pounds since the beginning f the year but have been trying to l
ose weight and dieting.

No palpatations at all. I have not had any symptoms really and was kind of surprised.

resting PR runs between 70-80 majority ofthe time. I check my BP every few days as I am on meds. Current check just now was 84

I do feel anxious now but mainly over this test result. Other than that no more so than normal.

No stool issues, regular good bowel movements

I did have strep throat the beginning of arpil coupled witha sinus infection.

Do not have t3 or 4 levels

Dr did palpitate my thyroid at last visit and said it felt normal. He commented that thyroid felt normal.

Get investigated

Dear Friend.
Your no signs or symptoms are suggestive of Hyperthyroidism.
But since your TSH is low, i advise you following:
1. FT3, FT4 and TSH
2. Ultrasound Thyroid.
3. FNAC - If Ultrasound is suggestive of any thyroid illness.

I personally don't treat if there are no signs and symptoms and even ultrasound is normal.
But i keep a watch on thyroid profile. I get it repeated after 8 weeks, if i find further change in values and i also notice change in clinical signs and symptoms, i start my patient on treatment.
I would advise you to first get the above said investigations and review with me, i would be in better condition to tell you that you require treatment or not if i have these investigations.
