What Does Increased SGPT Level Indicate?

Question: My serum vit d level is 3.09 ng/ml; ca- 10.4;k- 4.3; na - 143 sgot(aST) - 47 u/l;sgpt(alt) - 135 lipase - 73, and bilirubin direct - 0.3. What is wrong with me?

Thanks for writing to HCM

I have gone through your findings in detail.
Increased SGPT indicates liver injury.Vitamin D level is also decreased.
It is important to know total bilirubin level and PT (Prothrombin time).

You also need few more investigations like routine hemogram,RBS(Random blood sugar),Lipid profile,Renal function test,Viral markers,Ultrasound of abdomen.Further investigations can be done if needed.

You should avoid fatty food.
You can take tablet Ursodeoxycholic acid 300 mg twice daily for three months.It will help in regeneration of liver cells.
You should also take vitamin B complex capsule once daily.Vitamin K supplementation depend upon clinical conditions.

You should also take vitamin D supplementation.Vitamin D is available in XXXXXXX form.One XXXXXXX should be taken weekly once for 8 consecutive weeks at night with XXXXXXX warm milk.Then one XXXXXXX should be taken once in a month for four consecutive months.You should also take calcium rich foods and expose your self properly to sunlight.

Rest of the findings are normal.

Sir, the tests were done because I stopped taking sizodone 0.5mg and attentrol since last 30 days which took for last 2 yrs with in spiral for ADHD.as requested by you,bilirubin total-0.7mg/dl,bilirubin direct 0.3;bilirubin indirect 0.4;Total protein-7.8;albumin-5.1gm/dl,globulin-2.7 gm/dl;albumin:globumi1.9;;sgot(AST)47u/l,sgpt:(ALT)-135u/l;alkaline Phosphatase-193 u/l,Lipase serum-73u/l,Amylase-83u/l,blood urea nitrogen-15.4mg/dl,urea-33mg/dl,creatinine-0.9,t3-2.9nmol/l,t4-113.3,Free t421.43,HB estimation-15.4,rbc-5.16,Pcv-45.2,Mcv-87.6,mch-29.8,mchc-34,platelets count-3.10 lakh/cmm,tc-6.4,monocyte-01%,esr-10mm,prothrombin time(PT)-citrate plasma test plasma-13.8 sec,control plasma-11.6 sec,prothrombin index 84,INR-1.18.TSHSerum-0.285mIU/ml.

Thanks for writing back

All findings are within normal limit except SGPT. Increased SGPT is related to Sizodone and Attentrol intake for two years. Sizodone and Attentrol causes liver toxicity in some patients.
It is good that total bilirubin level is within normal limit so it is not in favour of viral hepatitis.
You should consult your doctor and you may need another drug or dose adjustment.
You can also take tablet Ursodeoxycholic acid as discussed earlier.
