What Does "he Inferior Aspect Of The Right SI Joint Has Marrow Edema.." Mean?

Question: What does…Impression: 1. Right SI joint has mild marrow edema inferiorty,at sacral aspect. It is not classic for sacroilitis, and there are no other findings of inflammation/arthritis? What is the above really saying in layman terms? Also found in Muscle & Tendons… Finding; Right more than left tensor fascia XXXXXXX muscle has a moderate amount of fat deposion within it. ? and here’s another Finding:… in bones & Cartilage.. The inferior aspect of the right SI joint has marrow edema at its sacral margin,with some adjacent low signal cortical and trabecular scerosis…What?… This one above…I really want to know exacally what they are trying to say? (layman terms- PLEASE DOC?)

Brief Answer: you are having sacroillitis Detailed Answer: Hello, I have read your question in detail and I will try to be as simple as possible in my answer. Also you are free to answer any followup question if you think that i missed some thing or you are not getting some points. So i will start now. 1. Your sacroilliac joint, which is present at lower back, just behind hip is showing some water collection in bone. This means there is some disease activity going on. Although it is very little, so you may not feel direct pain or any pain on pressing on your back but your thigh pain can be due to this fluid. 2.Your thigh muscles are showing fat deposition, that means your thigh muscles are getting weaker and at the place of muscle tissue there is fat. So if you stand for long time, you may feel tired a little. 3.There is sclerosis at SI joint means there is some sign of your old AS as well. And i can assure you this does not mean tumor. Whenever above mentioned fluid in joint get dried up it become little sclerotic on MRI. So its kind of change which shows that a healed disease is there. 4. Hamstring tendinosis is pain and swelling in muscles of back thigh. This can happen during playing any sport, doing yoga and doing any activity which put strain on thigh muscles. .So this explains your deep pain in thigh . 5.Your hip joints are normal, there is no water, or calcium deposition or loss of blood flow in hip joints. They are in very good condition. 6. Atrophy of tensor XXXXXXX muscles again mean that your thigh muscles are getting weaker. So if I summaries all MRI finding it means that there is some weakness of your thigh muscles, and there is some fluid in your SI joint. So there is nothing very serious or alarming about it. There is no tumor or any such thing which can become a serious issue in future. These findings are reported even in some normal person as well. I would like you to suggest that for time being do not do any yoga exercise as it may cause thigh muscle strain. Take protein in diet like egg, fish, chicken. For pain relief you can take either Amytryptalin and tramadol after consulting with doctor. Also apply warm water over thigh( you can sit in a tub) when you are taking bath. It will reduce your pain. When you say healed disease..Does this mean AS in not active anymore? or something else was happening- I did not know about? 3. I was wondering about the Dormant-TB. Have you ever heard of it ever going away without treatment? You mention the Atrophy of tensor XXXXXXX muscles thigh muscles are getting weaker...What would cause this weakness? 4. I had ask question about bladder, It's not happening all the time- but I call them "episodes" that may last a day or two in a row and happens about 3 to 4 times - a month. Keep in mind I had complete hystorectomy years ago. I was wondering "if" what is going on can affect the bladder? I have pressure bladder filling full and always having the urge to void bladder- and still fills full after voiding. Bladder has achyness all day long (when it happens) urge of going and after voiding- no burning during? I have had urine checked for bladder infection- always negative? 5. I also have bone spurs that show on Xray in neck and lower spine can the spurs be affecting nerves- pressing on the nerves or pinching nerves and would this also cause numbness and tingling in ALL limbs? 6.If nerves are affected by spurs and perhaps can they distrub the bladder also from time to time and if nerves are being impinged. 7. Could I benefit from physical therapy of any kind? Also, the MRI Technique the way its writtenIf this test was done with contrast 7. What does contrast show... Would it show different results? 8. I also ask what kind of Specialist Doctor- should I follow up with about this- or should I ... Is there anything that they can do? Thanks Again for your time. I do take Vitamin D and Multi-Vitamin also I take B-12.. I will try you advice on Calcium also be aware of my protein take. Do you have Suggestion on how many MG's or Units to take of Calcium? Brief Answer: physio will help Detailed Answer: hello, i can understand your concern. Medical terms are bit confusing. Let me try to solve your queries. 1. When i say heal AS , I mean sclerosis suggest that disease in bone is not active, but in other part it can be in active state as it is multi system disorder. There are blood investigation like ESR which tells about disease activity in body. AS can not be completely cured, best we can do is to suppress the disease activity so it no longer affect muscles in body. This is called remission. Sclerosis is suggestive of remission of AS in your si joint. 2. Dormant TB is when disease is not active and it does not need any treatment. 3. Your bladder problem and your thigh muscle wasting can be due to nerve compression at back. I would like to get MRI of your spine if there is any nerve compression. 4.Yes, physiotherapy will help but first we get MRI to see extent of nerve compression . 5.Your MRI does not suggest that any contrast was used. Also there is no need of contrast if we are looking for SI joint or spine. 6.You can post your MRI spine report to Us also. If there is any nerve compression an orthopaedic surgeon will be able to help you. 6. In your age at least one gram of calcium should be taken. Thanks for asking followup questions.
