What Does Fecal Soiling In Pants By A Child Indicate?

Question: my 7 year old daughter poops in her pants daily. She can use he toilet, but doesn't. We have tried several approaches, rewards/scheduling bathroom time/removing privileges, but nothing has worked. Help.
