Question: my husband is 62 years old . he has myocarial infarction on 2010. he is on medication regularly. he is taking olmezerh10, clopidogral75mg, atorvastetin 10 mg , aspirin 75 mg, sorbitrate 2.6 twice daily and a vitamine. his EF is 42, blood reports are ok, HB% is 11.2. how to increase his ejection fraction, what will be his diet and lifestyle. he is retired now, and having belly fat. kindly help me doctor regards

Dear Madam

1. The ejection fraction (EF)of 42% is considered mild LV systolic dysfunction. The recovery of this low ejection fraction to normal depends upon many factors, most important of which is the actual cause of low EF. If a part of heart muscle is totally dead or it is not contracting well or there are electrical abnormalities like Left bundle branch block. When a part of heart muscle is totally dead, it is not possible to recover EF completely to normal. However if it is not contracting well, then interventions like opening up of some occluded vessel and certain medicines may help. If he underwent an angiogram and coronary intervention. However certain medications (which include olmesartan or olmezest) help in improving EF. Beside certain betablockers can help improve EF. You can discuss this with your physician.

2. Diet must be containing number of calories to maintain an ideal body weight (a body mass index of ~23). It should not have saturated fats like butter, hydrogenated oils etc. He should avoid sweets and fried items. He must take high fiber diet like fruits/ vegetables/ salads. High calorie dairy products like high fat containing milk/ cheese should be avoided.

3. He must undertake supervised exercise daily which can be tailored as per her physique and other parameters by the physician. A simple brisk walk will help such patients and is often the recommended form of exercise. Duration and intensity can be adjusted individually.
