Question: Hello doc,
A month ago I had protected sex with a sex worker.
On the 3rd day I got burning sensation on my pennis and severe back acke.
There was no puss or boils.

Urologist prescribed offlox 200mg twice a day.I took it for 14 days.
The urine culture test came back as negative.
The burning went away.
BT then it comes back in a slight way sometimes which goes of with good intake of water.
The doctor gave me doxcyline 100mg once a day for 15 days and fluka150 tablet once a week.
Now I have got continuous burn on my annus.
I am applying proctosedyl cream for that.how to get rid of this burn in annual?

Why has the burning sensation came?
Is it a std?
Do I need to take a HIV test?

Dear Friend
Welcome to HCM. I am Dr Anshul Varshney. I understand your concern.
On basis of your history it doesn't seems to be a STD.
Probably you have got the burning due to friction during the act.
There is no relation of having sex with anal itching until and unless you had some activity with your anus.
You can continue with that cream.
As far as your urine burning is concerned, since you have already taken antibiotic and your culture is negative, and if you don't have any fever, then you can wait.
This symptom would improve in a few days.
After seeing the photograph, a possibility of fungal infection is there, that can also cause urine burning.
I advise in these situations local application of Miconazole cream and a single stat dose of Fluconazole 200 mg.
There is no need of getting HIV tested as the report would be negative at this time.
If you have any further query please ask me.
Stay Healthy.

Detailed Answer:
Dear XXXXXXX
I understand your concern.
These 2 tests would cover HIV and Syphilis.
If you wish you can get them.
I know you are more worried for burning in urine.
Many a times this is due to some stone in Urinary tract.
So also get an Ultrasound Kidney and Urinary Bladder.
Start taking Alkasol 10 ml thrice daily in half cup of water for 3 days. This should help you with burning in urine.
So follow these advise and revert back to me after 7 days.
Mention my name on your query always so that it is redirected to me only as I know your case completely.
Good Luck.
