What Does A Painful Bruise On Innder Thigh Indicate?

Question: Hello,

I am 38 year old male. Due to a car accident in 2000 I have been on pain meds to treat daily, debilitating headaches. For the last several years I have taken the "new" opiate Nucynta 300-400 mg/day as well as 90 mg of Temazepam (PTSD - long story) I take the Temazepam throughout the day with my Nucynta. I know Temazepam is for sleep, but believe me, my doctor and I went through everything and somehow it just works and I don't get sleepy on it. It just works with the Nucynta to relax me so i can stay calm (history of panic attacks and anxiety).

Anyway, a year and a half ago I got extremely dehydrated for about a month (work outside in the heat). I didn't realize how dehydrated I got until I saw my Doctor for minor hip and inner thigh pain. We ran test - everything came back perfect except my Kidney function was a bit too low and my egfr was 48 - My Dr said was a combination of my meds and just not getting enough water. I was outside all day and got very hot over long periods of time and did not drink enough water. I got control of that, that was the end of last year. I stayed hydrated and in Feb this year the follow up blood work showed everything back to normal - Kidneys came back fine. Liver fine. He does a ton of lab on me every few months because of my Testosterone injections ect.

WellI got a little dehydrated again for about two weeks last month (not as bad as last time and urine has always been perfect. Not brown, not clear, just a perfect light straw color every single time and it does not hurt to urinate like it did last year) But I recently felt that inner thigh pain come back on again. (not as bad as last time but it seemed like the same thing was starting over.) So I got hydrated again and all that is returning to normal. I have lab work in a few weeks.

So all that to say this, I was sitting on the toilet last week and my inner thigh (right inner thigh - same place as before ) was bothering me a little and I noticed that I had a very small (smaller than a dime) yellowish bruise and it was painful to the touch but no more than any other bruise. And it is all but gone now but it seems to have stayed around a few days longer than I would have expected but that may not be true.

I have No swollen lymph nodes. I have had two a major oral surgeries this month and probably didn't drink enough. I am scared (probably irrationally but nevertheless scared I might have lymphoma (although other than the inner thigh bruise that has hung around for a while but it's going away.)

I take testosterone injections from my doctor 200 mg 2 times a month.

Some people say it's the narcotic (nucynta) causing my prob.
Others say it's just a bruise ( I do have a history of incorrect self-diagnosis ;)
Others say that my kidneys may be stressed again

I know you can't officially diagnose me without seeing me and I won't hold you to anything. Just would like an opinion. Is this panic time or not? I had a full broad amount of blood work done in Feburary this year and everything - including my liver and kidneys were normal. I have GERD, as well which as recently caused my throat to feel time like someone is pressing on my XXXXXXX apply making it harder to swallow. I've got an appointment with a Gastro to do a scope and stretch my esophagus (had this done before and it feels like the same symptom and that fixed the throat, but I wanted to tell you that I am experiencing that as well right now).

Sorry for this long letter... Can you give me your opinion on if I should panic or not lol... The inner thigh bruise seems like I might be blowing it off. But I've had two of them over the last month. Same place, same size, same duration. I could have easily done something to it but I don't recall what that would have been.

I have gone through your query with diligence and would like you to know that I am here to help. In most cases, we doctors always complain about never getting enough information from our patient because they just did not feel it important or relevant enough to mention; well no complains here at all sir. :)

The first thing I would like to emphasize on is that this is the only bruise mentioned by you, and it is not that you have been experiencing multiple unexplained bruises, so that means you are at no risk at all.

Secondly, this bruise seems to be healing and slowly disappearing according to you so that is also great news. Now to help you with understanding this bruise, which in my opinion is most certainly a bruise. A superficial bruise to start off with can be black or blue in color, but as it heals this can change in to different shades of green or yellow. And when a more deep seated (internal) wound or bruise takes place it presents in the form of a yellow patch or bruise superficially. Given the history of moving stuff around, and the added suspicion raised by your wife whom I trust entirely, this is a result of some wound or injury sustained during your physical activity sir. Your presentation does not match that of a lymphoma in any way. So please do not worry about this.

As for the GERD sir, a sensation of a lump in the throat is one of the symptoms of GERD along with difficulty swallowing. You have not mentioned any medications to control your GERD and so I would strongly recommend the same. An endoscopy would obviously help, and during this I would like you to request your doctor for the status of your lower esophageal sphincter (LES). Depending on the status of your XXXXXXX we can decide the way forward for your treatment. If at all no treatment has been initiated, then I strongly advice the same.

Thank you so much you have put my mind at ease. After i submitted the question something occurred to me (probably of no significance) but about 3 weeks ago i had a similar small yellowing bruise in basically in the same place on the other (opposite inner thigh) it was there but it was small as a dime and yellow and didnt hurt unless i pushed on it. It healed and disappeared. I thought nothing of it.
Bottom line in the last month i have had two identical dime sized yellow bruses one in roughly the same place on both sides of each inner thigh and prolly a similar bruise on my shin that was yellow and went away normally. Sorry for not mentioning that in my original presentation but they seemed like such a non-issue I thought nothing of it. Everybody gets bruies so i thought nothing of it. Anyway all three of my bruises Were small and healed like any other bruise. The only thing that bothers me is that i had matching ones on each hip for few days but they were not lumpy just small yello bruises that went away. I just dont know where i got them. and it was on the opposite inner thigh in roughly the same place. but it didnt seem significant at the time (and it probably still is not significant) It couldnt have been there more than 10 days or ao. These are NOT huge hard purple and black things. was such a non issue it only occurred to me now. it. It was so minor and disappeared so quickly. One ther thing i forgot to mention is that about every summer. (Allergies) i tend to get itchy all over my body after i take the nucynta. But the nucynta does teigger it. He gave me a short course of Prednisone and did bloodwork. The Prednisone knocked it out and this year again i starting itching all over my body a couple weeks ago. (about the same time as last year this same thing happened. So i am in the middle of a short course of prednisone right now - because i have pins and needles i feel all over my body but my bloodwork is fine and my Dr. thinks its a hystamine allergic response to the Nucynta opiate. After the prednisone course last year the itching was done and i resumed Nucynta with no problems when the itching returned a couple weeks ago and the Prednisone is knocking it out again. Im fine in the morning its just something that seems to effect me in laye summer/fall. In combination with nucynta. . Anyway, so when the nucynta brought on the whole body pins and needles again this year but the prednisone will fix that. I dont think any of this will change anything but full body itching. I should tell you. Dreaded opiate itch? Do you concur with my other dock this is a hyatamine issue?

I cannot say that your doctor is wrong, but at the same time I cannot completely agree with him. Reason being, that when more than one unexplained bruise is seen on the body we as doctors have to rule out all possible conditions that can cause such a presentation, and of course allergies are one of them, but it would be in the patients best interest to rule out all other causes as well.

For your reference, I shall list out all the causes for explained bruises on the lower extremities. They are:
-Injuries (possibility)
-Diabetes
-Vitamin deficiencies
-Reduced body fat (recent weight loss?)
-Aging (unlikely)
-Side effects of medications (as suspected by your doctor; a strong possibility)
-Over exposure to the sun
-Grave's disease
-Blood diseases (eg. hemophilia)
and a few other less common and less likely causes.

Please take up all these conditions with your doctor, to make sure allergies are the cause for your presentation and nothing else. I only wish to be thorough and rule out the worst before assuming the simplest of possibilities. I hope you find my response helpful. Thank you. Ive only had two "unexplained" inner thigh bruses and they were small and left fast. The one on my shin my 2 year old nephw kicked me there so i know what cause that and im 90% sure the other two (although at separate times this

No sir, I do not think this is lymphoma. I am sure you are aware about the saying, asking you not to believe everything you read on the internet. Although the bruise can be one of the presenting symptoms of a lymphoma, I strongly doubt this is lymphoma sir.

So please do not get worried, and spend your time and life stress-free. In 3 weeks time, I am sure your doctor can physically examine it and rule out lymphoma completely.
