Brief Answer:
it can cause bleeding

Detailed Answer:
HI, thanks for using healthcare magic

The main possible complication is the risk of bleeding at the site of trauma. It does not increase the risk of fracture but because of the anti clotting can increase the risk of bleeding.

In most cases, if bleeding in the soft tissues was present there would be discoloration of the area, initially red and then as the days progress, it would change in color as the blood is broken down

He may need an x ray if the pain in still severe though pain tends to increase to its peak 2 to 3 days after a traumatic event and remain at that level for a few days.
Because of the xarelto he cannot use anti inflammatory pain killers such as ibuprofen or naproxen.

Summary-- Though cannot cause a fracture, can cause bleeding. It would be best for him to be assessed and may need imaging

I hope this helps, feel free to ask any other questions
