Brief Answer:
BNP level seems to be within the normal ranges for his age.

Detailed Answer:
Hello!

Welcome back on HCM!

Regarding your concern, I would explain that BNP level is affected by several concomitant conditions which should be carefully considered while trying to interpret the results.

In this case, BNP level of 800 pg/ml is within the normal ranges for the age group of 50 - 75 years (300 - 900 pg/ml). Correlations with symptomatology and other medical tests are necessary.

Hope to have been helpful to you!

Kind regards,

Brief Answer:
I could explain:

Detailed Answer:
Hello again,

This complaint and others need to be further investigated as I mentioned about by integrating a careful physical exam with all the underlying medical tests (cardiac ultrasound, chest X ray study, blood test for inflammation, anemia, etc.)

Answered by Dr. Ilir Sharka (10 minutes later)
Brief Answer:
Diagnostic tests and a timely applied therapy would be beneficial.

Detailed Answer:
Breathlessness while making minimal physical efforts like talking is an expression of advanced heart failure (if no other alternative causes are revealed).

In such case, there is urgent need for a timely diuretics regimen and a close hospital follow up to closely monitor the patient's overall hemodynamic conditions and potential complications.

I recommend asking immediate medical assistance in order to make the preliminary diagnostic workup tests and start an appropriate HF therapy.

Greetings!

Ok . For advance hf is it correct that survival is 12-24 months even with medication?

Brief Answer:
I would explain:

Detailed Answer:
Time prediction is only an approximation and not rarely does not properly correlate with each patient.

When considering such predictions, it is necessary to consider multidimensional correlations between clinical symptomatology, cardiac performance estimate and the spectrum of applied therapy.

It is difficult to make predictions only on a few available medical evidence.

That's my opinion!

One thing - dr today said stage 4.what is survival like fir that?
doctor
Answered by Dr. Ilir Sharka (2 hours later)
Brief Answer:
I would explain as follows:

Detailed Answer:
Hello!

The average survival for a patient at this stage is 5 years.

But as I explained to you before, with a regular follow up and an optimal therapy, these data can change drastically and the survival can be much longer.

Hope to have clarified some of your uncertainties!

Wishing all the best,

Dr. Iliri

Even with stage 4?
doctor
Answered by Dr. Ilir Sharka (1 hour later)
Brief Answer:
Opinion as follows:

Detailed Answer:
It is very likely for the life expectancy to improve if the reasons of CHF decompensation are a sub-optimal medical therapy, a poor medication adherence and the absence of a regular medical follow-up program.

I would not prefer to make any exact prediction, as it would not be ethically correct and from the other side nobody would be possible to predict if it would be 1, 2 3 or more years, judging only on some clinical symptoms, CHF degree quantification or EF estimation.

As I have explained you before, there are many actors on the scene, and the way how they interplay with each other may determine also the issue of your concern.

Best regards,

Dr. Iliri
