What Do These Ultrasound Findings Indicate?

Question: I am a 59 year old female diabetic on metformin about 99kgs in weight 1,7 m tall. Last month I had a sharp abdominal pain to my lower right side, I consulted w a gp who suspected CCF. He prescribed elanapril and I reacted badly in a day to it. I then changed tablets and once again the sharp pain started and had fever, the GP suspected pyelonephritis and put me on iv antibiotics.My sugar levels were also too high ranging 12 to 18 mmol and they injected some insulin. I have gone for chest xray and they say right side of the heart is slightly swollen. Abdominal scan was like thisLiver measure 15cm slightly enlarged and prominent, no focal lesion or cyst seen, kidneys both appears prominent measures L 11.5 × 5.7cm R 11.6 × 5.7 cm , Urinary bladder walls thick with echoes, uterus appears heterogeneous showing areas of mixed echoginicity suggestive to be fibrotic changes. The pain is still unbearable and I seem to have no appetite . What does the scan mean

Mild hepatomegaly may be due to many causes.

Hello

Your ultrasound findings suggests mildly enlarged liver(hepatomegaly) and it may be due to many reasons like hepatitis,fatty liver,metabolic disorder etc.
In your case it may be due to CCF as right side of heart is also mildly enlarged.
This may need proper clinical examination and further investigations like ECHO.

Findings also suggests changes of cystitis(urinary bladder infection).Cystitis is indicated by thickened urinary bladder wall and thick echoes in the urinary bladder.
Uterus also shows changes of fibrosis and it may be due to past infection.

These are not serious findings.This is a manageable condition.

I have also just done urinalysis and the lab tests show crystals and some casts wat could this mean

Since two days ago they have increased my metformin dose to 1000mg twice a day , but my sugar levels are not dropping yesterday it was 9,8 mmol today it's 11.1 fasting blood sugar. What advice can yu give me to lower the sugar. I am also on antibiotics for the urinary infection ands, furosemide. The pain on the right side does not seem to go away. The doctor had suggested I don't take paracetamol but the pain is unbearable sometimes.

Hello

Blood sugar level generally shoots up in infection,this is natural.
However,you need close monitoring.If it rises further then you need to take additional hypoglycemics like glimepiride,glynase etc after evaluation.
I don't think there is any problem in taking paracetamol.
