What Do These Lab Reports Indicate?

Question: hi,
right now my daughter is 10 months and 12 days old.she had cough, mainly congestion from may 07 till may 22.she was given levolin syrup,relent syrup,tab. montair 4 mg(see attached prescription).it was cured.

then from may 31 she is having fever.till XXXXXXX 02 it was 100-101 deg. from XXXXXXX 03- XXXXXXX 04 it was approx 102.now from XXXXXXX 04 midnight it is 103-104.initially it was managed by crocin drop but now she is given crocin ds followed by meftal-p because crocin ds does not breaks the fever even meftal p lowers the fever for 4-6 hours.doctors has prescribed some blood and urine test.reports is attached.kindly tell me what do you suggest the reason behind high fever.my daughter was feeding well and playful when she did not have fever till yesterday but now from today she is not taking proper feed and she cries suddenly in between.kindly go through all attached reports and tell tell me whether i should go for some new tests to find reason for high fever and do u suggest some more/different medicine.what would be the reason for such high fever?

Looks like urinary tract infection - suggestions given.

Detailed Answer:
Hi.... I understand your concern. I have gone through all the details and the attachments provided. By what you say I feel that your child is having a urinary tract infection and the reports are also conclusive of the same.

Coming to the last episode of cough and cold the kid might have come out of a viral lower respiratory infection.

But the present episode with high spiking fever and more pus cells in the urine along with nitrites and leucocyte esterase being positive, is very much suggestive of urinary tract infection.

My suggestions -

1. Please do not use mefanorm as it contains mefenamic Acid which is not good. Mefanorm contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run.

2. I would not have started an antibiotic like Cefixime, without doing urine culture or blood culture. Even now, I suggest that you do a urine and a blood culture now and also to do ultrasound abdomen to see if there is any dilated ureter.

3. Please get back to me after doing this with the culture report and I will guide you further.

thanx for prompt reply.
as suggested by you blood and urine culture report will come tomorrow.
from last 10 hours her temp. is 100 degree and we have stopped crocin or metaform.she is feeding little less like 60% of earlier feeding routine.

PFA new report!
1) pls tell me whether there is need for hospitalisation on the basis of previous blood and urine reports and now recent reports?

2)is antibiotics manipulating/suppressing actual condition or she has improved after looking at recent reports?

3)do you want to add any new medicine or test?

Hi... I have seen the new reports​ and compare them with the old ones.

1. I do not take the decision of admitting a patient based on the reports alone. Most importantly the general well being of the patient is crucial and as you say the fever is getting better and the child's oral intake has improved, I think she can be managed with oral antibiotics. I do not see an immediate need for hospitalization.

2. Antibiotics might be responsible for the change in the blood picture in the investigations​. But personally I would not have preferred to do this investigations of CBC, CRP, liver enzymes and a urine test as,ast would not change my line of management.
Please do get back to me with the final urine and blood culture reports​. But I am afraid it might not come positive as antibiotic has already been started. But we should give our best try and that is what we have done.

3. I do not suggest any new medication or blood test as of now.
