Question: Can you please restate what doctor has written in the discharge report. I cant read it so please rewrite it for me.
Also help me in understand its meaning in layman terms.

As per your query answers are-
Cardiologist has written that patient was admitted with complaint of having chest pain.After that angiography test had been done which revealed that one out of three main coronary artery was blocked.
Hence bypass surgery(parallel vessels are implanted along the blocked area) was done after surgery patient was stable and now is being discharged in stable condition.

In lay mans term the artery blocked Left anterior descending and ramus intermedius(they are name of two different pipes) as there was a blockage a vessel name LIMA(Left internal mammary artery and right saphenous vein has been implanted) parallel to blocked zone to maintain blood supply.
When we have a block on highway we use parallel roads to avoid jam same principle is applied here.As main artery is blocked parallel vessels are implanted.

Hi again,
CABG means bypass surgery.Coronary artery bypass surgery, also known as coronary artery bypass graft is a surgical procedure to restore normal blood flow to an obstructed coronary artery.

When there is a blockage in coronary artery to by pass it we need a spare artery in body of person.LIMA stands for left internal mammary artery its a spare artery present in chest and is used as a graft(as a bypass for blockage).

For the second graft we use another vein present in leg which is practically of no use its called Right saphenous vein.

OP means operation.
