What Do These Following Lab Reports Indicate?

Question: The patient had DOE since few months post which @ D echo done and angioplasty cone .Repeat @ D Echo did not show any improvement in LVEF.
need to know what is the best option available .

Not much options

Detailed Answer:
After going through all the details and attached reports of the patient, long term prognosis does not looks favourable.

Reason behind this is that
1. Severe LV dysfunction
2. Major part of the myocardium is not viable as per cardiac MRI reports.
3. LV has already dilated

To make the patient symptomatically better the dose of the diuretics as well as ace inhibitors can be titrated up slowly keeping an eye on patients blood pressure as well as renal and electrolyte status.

As the patient is young so Cardiac Transplant could be an option if affordability is not a concern. For this decision opinion of a team of cardiac surgeon will be required.

AICD can be useful in this patient only as a precautionary measure to prevent or abort any life threatening arrhythmia finally preventing sudden cardiac death.

But we should bear in mind and explain to the patient that it's not gonna improve the cardiac function dramatically.
