Question: Hello Doctor, I am 32 year old Male. On 29th May 2015 I had an unprotected insertive virginal exposure with a CSW. It was a brief exposure (less than a minute) with no ejaculation inside the virgina. I was extremely concerned after this event and was very nervous & anxious throughout. I was unable to sleep well at night and my mind kept thinking about HIV & STIs I could have caught due to my mistake. 10 days later I experiences mild lower abdomen cramps and burning sensation during urination for which GP gave me medication for UTI. Urine test and culture showed no bacteria, though I have to admit that I appeared for test after taking the UTI antibiotic at night. After 2 weeks of antibiotics the symptoms of UTI came back, also started feeling groin pain, leg joint pain & gas issues. I visited a urologist who prescribed semen culture and analysis. Semen culture showed no pathogenic organism but semen analysis revealed PUS CELL 12-14/HPF. Urologist concluded I had abacterial prostatitis and over active bladder for which he gave me bactsol and Roliflo OD. I should also tell you that I was already taking medicine for Hypothyroid since March 2015 (Thyronorm 50mcg) and was having difficulty controlling urine since mid 2014 for which I didn't consult a doctor & kept ignoring it. After starting medication for Prostatitis my physical condition improved but my anxiety was not going and I was having acidity & gas issues for which my urologist prescribed antacid & advised me on lifestyle alterations like slow eating, 8 hours sleep, yoga, meditation and exercising. After 3 weeks on medication and following lifestyle advise my condition became better. However fear of STI & HIV was not allying and I did following tests.
2 weeks from exposure - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE
3 weeks and 4 days from exposure (25days) - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE
4 weeks and 5 days from exposure - STI test (hsv 1+2 IgG & IgM, anti HCV, HBsAG Qualitative, Hepatitis B Profile, Chlamydia Trachomatis IgG & IgM, VDRL Serum) . RESULT - ALL TESTS NON REACTIVE and reports reviewed by my Urologist who had physically examined me and told me earlier that I had no STIs but wrote these test on my request.
6 weeks and 3 days from exposure - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE
10 weeks and 3 days from exposure - HIV Ag/Ab Combo-CMIA(ABBOTT Architect) Dr Dangs Lab . RESULT - NON REACTIVE
11 weeks and 6 days from exposure (83days) - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE
13 weeks and 3 days from exposure - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE
17 weeks and 3 days from exposure - HIV Combo (Roche COBAS) & QUANTITATIVE ASSAY OF HIV-1 RNA Dr Dangs Lab. RESULT - NEGATIVE
18 weeks and 5 days from exposure - HEPATITIS C ANTIBODY Quest Diagnostics. RESULT - NON REACTIVE
24 weeks from exposure - HIV 1&2 AB CHEMILUMINESCENCE Quest Diagnostics. RESULT - NEGATIVE

Kindly advise do I need further testing for HIV or do I take my tests as final & conclusive that I am free from infections. Getting confused if 6 months means 24 weeks or 180 days. Since 29th May 15 I have had no risky sexual encounters at all.

PS- I am feeling completely normal otherwise and subsequent visits to Urologist after taking medication for Prostatitis showed recovery & doctor had put me off medication after 3 weeks course & advised me to continue Roliflo OD till October. I have stopped roliflo after feeling normal and now only medicine I am taking is Thyronorm under GPs guidance.

Brief Answer:
No STD or HIV. No further tests advised.

Detailed Answer:
Hi
Thanks for your query at the HCM.
I understand your concern and situation.

Your case is not that complicated as you are thinking. I assure you that you have no HIV or STD infection. You need no further testing for them.
In your case you had developed genitourinary tract infection only. If you still get problem I advise you complete hemogram, ESR and Kidney function test. Get 3 days consecutive early morning midstream urine sample and semem sample tested for gram staining, wet microscopy and culture both for bacteria and fungus. Also get urine leukocyte esterase and nitrite test with urinalysis done. USG abdomen and pelvis also advised. Then depending upon test results I would advise you treatment. Drink plenty of water. Continue thyronorm and prostate treatment as advised by your doctor.

Take care

Hi Doctor, do I need to get any specific test for HIV 1 Sub Type O. Confused whether QUANTITATIVE ASSAY OF HIV-1 RNA test and HIV 1&2 Ab CHEMILUMINESCENCE look for this rare sub type as well.
