Question: Need opinion about my USG and semen analysis reports

Thanks for writing to us.

I checked your USG report and semen analysis.

In sonography there is no finding suggestive of tumor. It clearly suggests decrease in size of testicle due to injury in past which correlates information you provided.

In semen analysis there are enough sperms (that is good) but problem is there with motility and agglutination. At least 50% sperm should be well motile to stamp it normal report. As well as there are pus cells which suggest infection. Infection can affect sperm motility.

I do not feel any serious complication from these reports. There is need of antibiotics to remove infection which your doctor has already prescribed.

For inability to having baby:

I would suggest going for fasting serum free and total testosterone level.

You can go for semen analysis again after 1 month of completion of antibiotic course from infertility point of view which will likely to be normal.

- For infertility work up I want to know that are your wife's all investigation's normal?

- Do you know what is the best time during female menstrual cycle in which successful intercourse likely makes woman pregnant?

Thanks sir for your reply.... Currently my wife dint go for any investigation becuase i thought i should go first as far as female menstrula cycle that i have an idea or if you bit explailn further as well .

Pregnancy occurs when sperm meets ovum in uterus. Ovulation from ovary occurs roughly around 14th day of menstrual cycle (when menses begins count it 1st day and cycle of 28 days). Pregnancy can occur in +/- 2 days of ovulation.

For exact response and clarification please let me know her last three menstruation beginning dates.

Hi,

Thanks for reply.

Her menstrual cycle is roughly of 31 days of interval.

So her ovulation is likely going to happen from 14th to 16th day of menstruation beginning date.

During this cycle her ovulation is likely going to happen between 25th to 27th of September. So you should avoid sex between 21st to 24th September for better sperm density. You should do daily intercourse between 25th to 29th September to have more chances of pregnancy.

Hope you are clear about the best duration.
