What Do These Echo And Stress Test Reports Indicate?

Question: Hello Doctor,
I tested "mild left ventricluar Hypertrophy"on echo test and "mildly positive for inducible Ischemia"today. These were routine tests. I completed the 9 minutes 10 METS stress test without a problem. I am 46/ male/ smoker/ in good health/ exercise regularly. How serious are these conditions? Are they reversible? Can you review the reports and give a second opinion please? Thanks.

Hello ,I am Dr. Mody and I would be addressing your concern.
I went through all the attached reports and would enlist my observation .
1 ) You need not undergo any further test at this point of time . repeat TMT after 6 months
2) You have to stop smoking as that is the only major risk factor you have .
If you are over weight and have a family history of heart disease this is the time to start making life style changes .
check you lipid profile and sos treat if cholesterol / trygycerides on the higher side
3) Echo shows mild hypertrophy however the value of 12 mm is just boderline .
The most common reason for same is hypertension . so keep a tab on the same and try cutting down salt in your diet . just hypertrophy is seen I people who exercise and have an athletic built.
4) Since except smoking no major risk factors , Next time consider getting a stress thallium ( preffered) or stress Echocardiography whichever is available .

why didn't I suggest angiography , it's invasive and the radiation and XXXXXXX related risk are not justified in your case .

Hello Doctor:

I would be inetersted in knowing what exactly is wrong in my TMT report, the magnitude, slope of ST depression etc., so that I can read up on the matter.
My BP is normal. LIPIds are borderline. Tryglycerides are normal. I am also an runner, and have practiced for the past 14 years or so (though not regularly. I run Half Marathons so practice for it every year for a few months)
I do realise the effects of smoking and have promised myselves to quit it immediately.

As per the literature on web, The magnitude and slope of ST depression is important in assessing the abnormality, which I am keen to understand. Hence will request the same.

Hello ,
as I start I would like to state that in reporting any test , the clinical risk of the patient is always taken into consideration as it would predict the pre test probability.
Considering you as a 46 year male with a risk of smoking , your doctor is justified in saying that it's boderline positive and keeping you in follow up to prevent any future cardiac event
The same result in a 30 year female who is protected by her hormones and have no risk factor , the test can be neglected .

So yes both the amplitude and slope is important in saying that the test is positive and neither should be neglected.
In your test at stage 4 , there is up sloping and horizontal ST changes in inferolateral leads.
However similar fine changes are even there in your baseline ECG , so we can wait before you undergo any further test , as there is a fair possibit that the test is false positive .

This is something which you shouldn't Google as it's highly subjective and a specialist decision .
Google would add to unnecessary stress , Rest it's your wish .

Yes do try quiting smoking and carry on with a healthy life and I don't think there would be any need for a invasive cardiac evaluation in near future .
