Brief answer....explained below
Detailed answer....
Hello,
Thanks for the query.
I have seen your report and understand your problem.
Your report doesn't mention about the morphological appearance of sperms which is very important for evaluation.
Your report shows presence of pus cells which means infection.
When infection is present,it damages the surface of sperm as well as can damage the DNA also.
What to do.....
Get the semen culture done and then take proper antibiotic course for around 4 weeks along with anti oxidants.
Get the semen culture done after a month along with the morphology of sperms. Get it done from a very good lab.
It seems very difficult for you to become father naturally.
Be prepared to go for IMSI along with ICSI or may be ICSI only.
Best for you would be to consult infertility specialist and get managed properly in a professional way.
I hope it helps, however you are most welcome to revert to me for any further query.
Best of luck
Thanks

Hello,
Very frankly speaking it would be better to take antibiotic only after semen culture and that too for 3-4 weeks.
HEALTH OK satchet is all right but overall this treatment is not satisfying to me.
If I am in your position, then I would consult an infertility expert and then take the treatment.
Thanks
