What Do The Following ANA And CRP Report Indicate?

default
Posted on Thu, 16 Apr 2015
Question: Hello Doctor,

Apologies for not being able to respond earlier, I was traveling last week and couldn’t access the website. I’ve attached the ANA and CRP reports. Also below are all the answers to your queries.

The pain is not localized. Sometimes I have pain in the lower ribs and sometimes it is there on the left shoulder blade or the middle of chest. I do feel some tingling in the left hand ring finger sometimes, I even feel like this finger is numb when I am sleeping (like finger is also sleeping). The pain has reduced a lot now. On a scale of 10 with 10 being highest I’ve experienced, I’d say it is at 3 these days. The pain is not on the left shoulder region neither it radiates to the left arm or neck. As mentioned earlier, I am on Ayurvedic medicine these days. The Ayurveda doctor also mentioned that I am getting all these because of weak immunity system and weakness.

Thank you very much in advance for all your valuable time.

Regards,

XXXX

Brief Answer:
ANAs results are positive. Rheumatologist advise is needed.

Detailed Answer:
Dear XXXXXXX

No need at all to apologise.

I have gone through the reports you have attached. ANA results are positive but however at a dilution of 1:80 there is some possibility of false positives. But I would however visit a rheumatologist and seek his opinion as to it's interpretation and clinical correlation. CRP is very mildly elevated and for all purposes may be a variation of normal result.
I would also get a cervical xray in AP view and lateral view too to investigate the numbing in the finger. Also preferably avoid a pillow if you are using one and use it only to lie down on the sides, that too not a thick one.
Also I think it is better to stop the ayruveda drugs too for some period of time. For what were you prescribed?
And how is the fever diary? If possible just take a snapshot of the recording and upload.
I don't think it is a bony pain actually, the one you are experiencing in the substernal region. It must be due to peptic ulcers. Anyway if cervical xray is clear it would be better to see a gastroenterologist since you also have a history of peptic ulcers. None of these are due to weak immune system. Everyone gets a fever and infection now and then. If anything, the ANA result points to a stronger immune system which might be causing the problem. But that is the case if the result is clinically significant. Besides your blood tests too show a normal cell count, so don't worry about your immune system. If the pain gets too much continue Dolo 650mg 3 times a day and avoid alcohol during the time you are on paracetamol.

If you have any questions, do get back to me here.

Regards,

Pradeep

Hi Doctor,

I will visit a rheumatologist and seek his opinion. If possible, can you suggest someone in XXXXXXX

I will also get a cervical xray done.

The Ayurveda medicine is for the same reason - fever and back pain. Since the time I’ve started them, I’ve seen some improvement. The pain and fevers have come down.

The fever is normal in the morning, and during the evenings it ranges between normal (on some days) to 99.2.

I strongly feel that there is a relationship between my back pain and the fever. I’ve noticed that when there is less pain, I have less or no fever. But whenever I have more pain, the fever is a bit high. I do sometimes feel that I still have Peptic Ulcers, as whenever I eat or drink something spicy or something even more salty I feel burning sensation in my stomach. Same happens when I lie down in night and change positions.

I’ve stopped taking alcohol from last three months and am taking very light diet. Avoiding everything which could cause me trouble.

Another thing is that I have a bit of soar throat, in the morning I’ll feel a bit of sputum in my throat which causes coughing, once that is clear there is no cough.

Thanks,

XXXX

Brief Answer:
Do let me know after the rheumatology consult.

Detailed Answer:
Hi,

Yes do visit a rheumatologist. Let me know too about the consult and xray of the cervical spine.

You should continue taking paracetamol in case the fever doesn't subside or goes above 100 deg F. Good that you are avoiding edibles that would aggravate your acidity. If it has been long since you got a endoscopy done, get one done to see if the ulcers are there still.
For sore-throat you can use a syrup like Sal Mucolite which contains Ambroxol. Also it will help to do steam inhalation every 6 hours, daily for a period of 10 minutes. Drink more water too as it would make secretions less viscous.
As for a rheumatologist, I don't know anyone personally. But however a friend and colleague of mine suggested Dr Sarath XXXXXXX Mouli, KIMS Hospital Minister road in Secunderabad.

Hope this helps. Do get back in case of further queries.

Regards,
Pradeep
