What Do My Lab Test Results Indicate?

Question: Hello doctor....As advised we got Jak2 mutation panel and epo level tested. EPO level is 12.6 mIU/mL and V617F mutation in exon 14 of Jak2 gene was NOT DETECTED. exon 12 report is awaited. I have a concern now that if exon 12 comes negative...do we need more tests further?. Also one important point...shall we just discard him as a kidney donor and look for some other family member ?

He has negative jak2 mutation for axon 14 with normal EPO level. If his axon 12 mutation is also negative then it suggest secondary polycythemia.

So further investigation should be done to search the cause of secondary polycythemia.

He can donate the kidney after through investigation. No need to reject the donor just in the basis of secondary polycythemia.

As per my understanding of polycythemia (through Google) it is a progressive disorder and is a kind of malignancy and organ donation is an absolute Contra -indication in case of haematology malignancy. I hope it is not transferable in case of secondary polycythemia ?

Really worried and confused :((

It is primary polycythemia vera that is malignant proliferative disorder.

Secondary polycythemia is not malignant. It occur secondary to other reasons. So we can not say right now that it is safe or not safe.

First of all we have to search primary cause which leads to secondary polycythemia. If primary cause is not a contraindication for kidney donation then it is ok. He can donate.

Hello doctor,

Good Afternoon !

We met hematologist , he checked the patient clinically and said exon 12 is not required as EPO level is normal?

And said it is secondary polycythemia as there are no symptoms of any sort and he can go ahead with surgery . Wanted to take your opinion on this....thx

Yes his epo level us normal so its most likely due to secondary polycythemia.

It is not malignant disease.

If a hematologist examined him and said its OK then he can definitely go for surgery.
