Posted on Mon, 22 Jun 2015
Question: I am a 61-year-old female who recently went to a clinic for a check-up. My last examination was 5 years ago. I have experienced leg numbness and tingling from time to time. I am feeling thirsty and tired. What major problems can you identify? What subjective and objective information supports each of my problems? What is your assessment and plan for each problem? What medications and doses are appropriate ?These are my lab results. VS:BP 180/103 HR 73 RR 13 T 98.4 Wt 90 kg. Ht. Brief Answer:
Analysis of the case summary

Detailed Answer:
Good day,

Welcome to health care magic

Noted your concerns. From the list of problems listed and based on examination and lab findings, i have come to a conclusion that, You have

1) Type 2 Diabetes Mellitus ( uncontrolled)
2) Peipherel neuropathy
3) Dyslipidemia
4) Proteinuria
5) Renal impairement
6) Uncontrolled hypertension

High sugars and feeling thirsty and tired supports uncontrolled diabetes.
Feeling of tingling and numbness together with impaired monofilament test supports peripheral neuropathy.
High cholesterol supports Dyslipidemia.
+++ Urine protein and high creatinine supports chronic renal problems with proteinuria.

Tiredness could be also due to renal impairement assciated anemia.

To answer further, i need to know your current list of medicines ( unfortunately, they are not seen with this question).

I can further suggest following blood tests
a) CBC, ESR
b) Thyroid blood profile
c) Vitamin B12 and Folate levels.

Regards
Binu
