Question: hello doctor,
my wife is 18 weeks pregnant.
we recently had sonogrpahy of hers.
please provide your diagnosis.
reports are attached.

I have gone through your question and understand the concerns.
I have reviewed the report.
It shows severe oligohydramnios or severely decreased fluid levels around the fetus.
It can be present in congenital renal anomalies in the fetus.
I will suggest her to undergo a detailed anomaly scan by a fetal medicine specialist to get appropriate diagnosis.
If severe renal anomaly is present, then termination of pregnancy is advised.

hello doctor,
what could be the reason for the sudden drop in fluid levels around the fetus? as in the last sonogrpahy all was normal . last sonogrpahy did last month on 22nd XXXXXXX 2017.

Renal anomaly in the fetus likely

Hi dear,
I have reviewed the report.

Fluid levels around the fetus till 12-13 weeks of pregnancy is formed by the placenta, hence is normal even in the cases renal anomaly in the fetus.

Beyond 12 weeks of pregnancy, the amniotic fluid is formed mainly by the fetal urine, so if there is any anomaly in the renal tract of the fetus, that can lead to severely decreased fluid.

So, I will strongly recommend getting a detailed anomaly scan done to confirm the diagnosis and management should be done accordingly.

hello doctor,
thank you for answer.

Currently our gynaecologist has advised the treatment of providing amniotic fluid externally through saline after every alternate day and advised us to perform anomaly scan after two weeks.

Is this the right approach or should we perform anomaly scan immediately?

Please advise.

It is prudent to get the anomaly scan first and then decide.
This is so because if any major anomaly is there, then the termination of pregnancy can be done only upto 20 weeks of pregnancy.
