Question: Tremor during isotonic exercise (even after long period of regular exercise and at well rested state), This is always like this in my entire life at gym.
When I had a quarrel with someone, my legs and arms shake.
From time to time, my eye lids will shake.
Although, I feel smart, my memory is not good relative to my colleagues.
And my great concern is my grandfather (fathers father) is diagnosed with Parkinson's disease.
My grandmother is Alzheimer's disease. My grandmother 3 siblings are diagosed with dementia (2 of them hydrocephalus induced dementia, on of them Alzheimer's). There is one more sibling but he was died from heart attack at an early age.

I have seen a neurologist at a public hospital, but the average visit duration is about 4minutes in my country. I only can manage to have a brain scan, blood work and she looked into my eye with a light. I attached the blood work and brain scan (MRI). I have attached the blood work to website. But cannot manage to attach MRI. If it is possible, you can download the scan from this link. http://we.tl/EehpkMOxVV

What should I do in this circumstances? Do I have to do something?

Brief Answer:
Your Brain imaging is normal

Detailed Answer:
Hi, thanks for your query. Welcome to HCM.

I have read your concerns and shall answer accordingly. I have gone through the link and seen your brain imaging. I can not find your blood work. It is probably not attached here.

Your brain imaging is normal. No abnormalities detected.

The life long tremors and cramps which you are reporting is normal phenomenon. Muscles are exhausted after strengthening exercise so there will be tremors and cramps for few hours after work out in the gym. It can last for more than 5 hours. So this is normal.

When a person becomes angry or in feared situation there is a release of hormones called noradrenaline and adrenaline. They are responsible for fight-flight reaction and causes the tremors, perspiration, increase heart beat etc. It happens with everyone. Even with me. There is nothing abnormal with you.

Dementia can run in family but the prevalence is very less. Familial dementia starts at early age from 35 years onwards. If you have any problems with your memory, daily work, handling money, problems at occupation, forgetting names, familiar faces, disorientation with time and place etc. then only you can think of some abnormality.

Memory problems can also due to current depression and anxiety disorders if any. You simply think about your present situation and tell me about depressive and anxiety symptoms if you have any. Tell me about any recent stressor you have faced.

I think you are just over concerned about family history of dementia. You can regularly go for testing and psychiatric consultation if you have any depressive or anxiety disorder. If any, then with drugs you will improve and your memory problems will disappear. For, cognitive assessment, you can go for MMSE [folstein] and other neuropsychologicl evaluations like PGI memory scale, Luria nebraska battery of tests.

Hope I have answered your query. If you have further question then feel free to ask.
Wish you good health.
