According to your information and clinical picture, I can assume that underlying condition could be Paget's Disease of the Nipple or Eczema. In this scenario, we need to rule out malignant carcinoma of breast which can affect both nipple & areola and present with itchy & crusty nipple. Here, I suggest you to consult with ONCOLOGIST for clinical examination of the breast and adjacent axillary lymph nodes to make a concrete diagnosis. Relevant investigations like mammogram, biopsy of breast tissue or nipple, Cytopathology etc are required for confirmation. Proper treatment depends on the diagnosis of the case. Surgical treatment is preferred if Paget's Disease is diagnosed. Mastectomy with removal of axillary lymph node is the procedure of choice in such case. Other supportive therapy may be required as adjuvant treatment.
