What Causes White Patches On Foreskin Of My Penis?

Question: Its regarding white patches on my foreskin , its thicker and rougher than the normal skin. The only issue is I can't pull back my foreskin, this makes it hard to clean and even reduces pleasure during sex. Although I must say there nis no level of pain or irritation experienced due to it. After the patches started appearing the skin at the tip began to grow and narrow at the tip which causes the issue. I'm afraid not treating it lead not to spread to other areas inside the foreskin and onto the penis eventually. Please advise.

As per your description you are suffering from Balanitis Xerotica obliterans.
It is a chronic, progressive, sclerosing inflammatory dermatosis of unknown etiology.
Topical, Intralesional steroid and Immunomodulators are tried before surgical intervention.
It generally does not spread to other areas of body, although the lesion of prepuce progresses with time and lead to sexual dysfunction and even meatal stricture.

Although the picture might seem the skin to be inflamed the case is not, as the patches are inside part of the foreskin and because its narrowing causes it to look like that when pulled up. What medication or ointment should I consider?

Thanks for posting clinical picture. I have reviewed all your clinical photograph. It seems that photographs are taken at different time. Few of them are after application of betnovate C (A potent steroid)
After reviewing clinical photograph I made provisional diagnosis to be balanitis xerotica obliterans.
I will advise you to go for therapeutic circumcision, as sequelae of BXO is phimosis and unable to retract foreskin which will hamper sexual life.

In the mean time apply emollient and Fluticasone ointment twice daily and consult your surgeon for therapeutic circumcision.

All the pictures where taken today only. And I haven't applied betnovate c . kindly prescribe an ointment analogue in XXXXXXX instead of the chemical contents please.
I haven't used any medications to now
I don't think I'll be able to do the surgery in next few months. Is it possible to delay it till then... How long can I use the ointment. What's the dosage and period of usage.

white patch is also due to Balanitis xerotical obliterens and is not vitiligo.

I usually treat my patients with the following medicines.

Flutivate ointment twice daily, it is easily available in XXXXXXX
Ureative 3 lotion twice daily. you can continue Ureative 3 lotion for long time but don't apply flutivate for more than 3 week without supervision.

Difference in picture may be due to different view.

You can postpone the surgery as medical treatment will improve your condition a bit.
