What Causes White Coating On The Tongue And Loss Of Appetite?

Question: Hello Doctor,

This is XXXX from XXXXXXX XXXXXXX Sorry, for writing such a long email.

The incident happened on '26th Sep 2016' (recently) and when I was traveling in a 'city bus'. I was 'stabbed' by a sharp object not sure what it was.

I really don't know whether the object had 'blood' in it and the person was 'HIV infected'

But, I believe it was not a 'syringe/needle or blade'. Because, the object left a little scar (2mm or 3mm in length) and the wound was not deep enough. When I undressed I saw a 'tiny dot' of blood but it did not ooze out

A week after this incident, for 5 days I consumed lunch outside continuously including typical 'hot & spicy andhra meal' (which may have lead to Typhoid but not sure)

Exactly after 10 days (8th October 2016) I had the following symptoms

     (a)     Light-headedness lasted for 3 days and gone without any treatment
     (b)     Had very little rash just above 'elbow' but quite a number of XXXXXXX spots above and below 'Elbow area'
- Rashes disappeared within 5 days and never appeared again
- XXXXXXX spots usually disappeared between 3-5 days. New 'rose spots' started to appear again 'above and below' Elbow area.
- There were 2 or 3 XXXXXXX spots in upper part of body but I really had to hunt down for it. This cycle lasted for 6 weeks
      (c)     Joint pain (in legs)
      (d)     Stiff Neck
      (e)     Back ache (behind stomach - lumbarback I hope)

Exactly on 15th day my doctor (I was not in a position to explain about the incident due to rush in clinic) asked me to conduct the blood test

     (a)     MP (Card test)          - Negative
     (b)     Typhoid (Widal Test)     - 1:160 S typhi 'O' antigen (Positive)
     (c)     Dengue (ELISA)          - Negative (Platelet count was 1,65,000. Doctor looked at the range of Ab/Ag and said really nothing to worry)

He said I was affected by typhoid and I took tablets for 5 days or a week (I don't remember exactly).

     (a)     I applied cream for 'back ache' and pain disappeared in a day and never appeared
     (b)     Stiff neck - Was 'ON and OFF' for 2 or 3 weeks but now it's gone
     (c)     I applied cream for 'Joint pain' and lasted for a week (I don't remember exactly)

Exactly on 30th day, I myself went to lab and conducted 'HIV ELISA' which came back 'Non reactive' - '0.16' out of '1'

But, after 30th day I encountered the following symptoms

     (a)     White coating on back of tongue - Doctor provided pills for 4 days and white-coating disappeared and never appeared again
     (b)     Loss of appetite for 4 days. I slightly reduced the 'quantity of food' for 3-4 days and then it came back to normal
     (c)     Two bumps on back of tongue (both sides) which exists till date. But, it's slightly raised not bad as how we see in 'Google images'
     (d)     Dry lips 'on and off' for a month now

     NOTE: I did not notice whether 'white-coating' and 'bumps on tongue' existed right from the start. As a side note, my tongue never lost its taste


Other Reasons which may be a cause ( from layman perspective )

     (a)     I always had the problem of 'gas' and 'belching/burping' (for couple of years) and I never undergone any treatment for it
     (b)     On couple of occassion after this incident 'my father' informed me that I have got 'bad breath' when I speak or yawn which I did not recognise
     (c)     I roamed a lot during 'summer season' and even felt 'some pain' while urinating (not regularly though) during night time
     (d)     I am really stressed, worried and depressed. I've got nothing else other than 'HIV' in my mind


I am yet to meet a 'Dermatologist' and waiting for '11th or 12th week' to complete for another HIV ELISA Test.

I am not sure whether I'll survie? Should I live or not?

Expecting some reply before I go for '12 weeks' ELISA Test

I appreciate your effort in explaining your symptoms in such a chronological manner.

Your symptoms are those typically seen in typhoid. I believe you were well treated for the same.

The symptoms you experienced after 30th day appear non specific.

I would like to see the lesions you have on your tongue. If possible do photograph and attach it here.

I would like to know whether you had any fever in recent days, if yes what was the temperature ?
Was the joint pains associated or aggravated with fever ?
Do you have loose stools ?
Did you suffer with sore throat in recent past ?


Also, I would recommend a P24antigen assay or a Viral RNA analysis over the ELISA. p24 can be reliable within one month of exposure and Viral RNA within days. ELISA needs a minimum of 3 months to seroconversation (appearance of antibodies) to occur.

I did not experience the following

Constipation, Diarrhea, Fatigue or Severe fatigue, Nausea, Vomitting and Sore throat

a) I would like to know whether you had any fever in recent days, if yes what was the temperature ?

     At the end of 3rd week and 6th week I had fever. When I met doctor with reports he did not prescribe any tablet for 'fever'.

     He vocally suggested to consume 'paracetamol' or 'dolo 650' in case if I suffer from fever

     After looking into the reports, doctor had advised me to consume juice and coconut water to cool down my body

     I consumed 'coconut water' the next day (after doctor's advise) and came to office and spent my day in A/C (without covering my head, ears and body using sweater or similar)

     After 2 or 3 days I had fever, but I don't know the temperature and lasted for 3-4 days. I consumed paracetamol for 3 days

     May be drinking 'coconut water' + 'sitting in Air conditioned room' could have triggered fever

     VIth week

      This scenario is entirely different

      I was walking on the road to reach my office. A car splashed considerable amount of water on my body and disappeared

     Hence after reaching office, I had taken 'bath on cold water', 'dried my body with the little scarf which I had' and 'spent my day in A/c' (again without covering my head, ears and body using sweater or similar)

     The very 2nd day I had bit of cold which developed into 'running nose' and 'fever' and lasted for 4 days. I consumed 'paracetamol' and 'amoxycillin' for this but again I don't know the temperature

b) Was the joint pains associated or aggravated with fever ?

On 3rd day night (after symptoms started) joint-pain in legs was bit severe. I hardly slept for 3-4 hours on that night.

     But, from next day on wards it was somewhat manageable

c) Do you have loose stools ?

     No.

     As I had started taking pills from the end of 1st week (after symptoms started), 'pills' may have stopped 'loose stools' but I am not sure.

     Kindly request you to check the prescription attached

     At my age of 15, I had suffered from typhoid but if I remember correctly I did not have loose stools but I had joint-pains

As a side note there was a tablet and its name started with 'Gastro'. I don't know about the purpose of that tablet but it may be useful to you

d) Did you suffer with sore throat in recent past ?

      No. Otherwise I would have informed my doctor about this or at least before taking the first pill I did not have 'sore throat'

      Again kindly request you to check the prescription attached if any 'pills' stopped 'sore throat'

e) I would like to see the lesions you have on your tongue. If possible do photograph and attach it her

      I've got 5 little bumps (2 on right side of tongue and 3 on left).

     3 of the bumps are almost flattened. 1 of the bump is slightly raised comparing to the 'flatted ones' and another one is slightly larger.

     I had taken photos but it's not clearly visible. When doctor had a look into it, he was not much concerned about it

b) I read that 'ELISA' is not a confirmatory test for Dengue and its for HIV. If 'Dengue ELISA' test detect anti-bodies there is a chance that the person can be HIV+

     If the above sentence is true, in my case as 'dengue elisa' did not detect Ab/Ag considerably can we assume that 'HIV anti-gen' did not exists at that point in time

10th week of window-period is going to end tomorrow. 12 weeks will be completed by this Dec 20th

It appears 'Viral RNA' and 'P24 antigen' is bit costly. That's not the problem

But, due to demonetization problem I don't have that much money as cash. Most of the labs does not have the facility to swipe 'cards'

Can I take 'HIV DUO' or 'HIV ELISA' (again) after the end of 12 weeks ?

I had attached my 'reports' which were taken after 2 weeks and most of the 'prescription' sheets

Please review them and let me know about your opinion

It always depends on the HIV strain. So, we cant be sure when the seroconversion occurs in a person for sure. Its always said only after 3rd month.

b) I read that 'ELISA' is not a confirmatory test for Dengue and its for HIV. If 'Dengue ELISA' test detect anti-bodies there is a chance that the person can be HIV+

ELISA for dengue and HIV are entirely different tests. The antibodies produced in both infections are different. The test results are not connected.

Can I take 'HIV DUO' or 'HIV ELISA' (again) after the end of 12 weeks ?

Yes, you can.

But still I would recommend HIV RNA PCR as gold standard and "end of the story" test.
