What Causes Weight Gain And Tiredness?

Question: Weight gain esp in abdomen(after losing/keeping off 75lbs for 5+yrs), inability to lose weight despite diet/exercise, heat intolerance, lack of sweating, muscle weakness despite resistance training, strange spells(get so weak I have to lie down & within minutes I'm unable to lift or move my limbs & head due to total weakness, speaking & breathing takes effort bc muscles are so weak but I am completely alert, just feels like my body has been 100% drained of all energy sources, lasts from 15mins-1hr, food seems to help some but someone has to literally feed me during these episodes, usually have a headache after). Every 2 months I'll have a week or so where my muscles ache badly, esp the tops of arms & legs, they'll feel heavier than usual & I'm unable to lift the things I typically can. Also I now have a frequent urge to urinate, constipation with bouts of severe abdominal distention(comes on quickly & can grow for a cpl of days, sometimes so large it effects my breathing & im unable to wear my typical pants). All has gone on for years; no doctor has been able to diagnose it. My newest issue are deep horizontal dents across my fingernails(& 4 toenails), 2 dents on each nail, same width apart. I know this is a lot of info, but I believe it is all connected somehow. Please help; I'm so tired of being weak.

Weight gain and tiredness can occur in a number of conditions. Do you have diabetes or any other illness? Are you on any medictions? How are your menstural periods?
Any blood works so far? Do you consider yourself suffering from depression or anxiety? Smoking/alcohol?
How is your sleep ? Do you feel fresh in the mornings afer waking up?
Do you snore when you sleep?

Have you developed new strech marks ( pinkish coloured) on your abdomen or thighs etc?

All these informations are important when we analyze your symptoms.

Noted that you also have constipation and nail changes.
Few blood test are required,

1) CBC, and ESR ( to look for anemia )
2) Thyroid functon test ( Free T4 and TSH)
3) serum electrolytes ( serum sodium and potssium)
4) Fastin blood glucose
5)Vitanin D level

Your symptoms are common in Anemia, Hypothyroidism, and in hypoglycemia attacks. It may even be worthwhile checking your blood glucose during one of these tiredness episodes. Vitamin D deficiency also can cause muscale weakness.
Obstructive sleep apnea and snoring can cause tiredness too.

I am on Adderall XR 30mg once a day, have been since August 2007. I do not smoke, never have, I drink once or twice a year. No new stretch marks. I also take a digestive enzyme before meals and a daily vitamin. I don't snore, no sleep apnea but I don't sleep well. I fall asleep but wake up several times a night. I tried taking 5mg of melatonin, it kept me asleep for 3 straight hrs which was great but then I woke up & stayed awake for the rest of the night.
I am not diabetic that I know of. These episodes have been happening for several yrs. I had a lot of different things checked in 2010(thyroid, A1C for blood sugar...everything was normal). During a cpl of episodes in 2010-2011 I was able to check my blood sugar(my late husband was diabetic so I used his monitor). One time it read 82 during an episode. Another episode it read 67. Also in 2010 I went to the ER during an episode. My muscles were too limp to walk on my own, movements were slow, I looked & felt drugged, speech was slow, it felt like my body was paralyzed. That's how lifeless & heavy my muscles were... They found nothing, sent me home & suggested I see a neurologist. I saw a neurologist a cpl month's later, on physical examination he said I had weakness in my hips, neck & arms. He did bloodwork which showed CPK levels almost 20 times the norm. The retest was the same. A month later he tested me again, levels were about 5 times the norm which he said was ok since my levels had decreased. He then dismissed me as his patient. Next I tried a rheumatologist. They ran bloodwork, a sleep-deprived EEG, an MRI of my head, an EMG & eventually a muscle biopsy from my thigh. Nothing was found. I stopped pursuing answers at that point & decided to just deal with the issues & episodes on my own. That was 3 yrs ago, & they're still happening.
My periods are typically painful, can be very heavy. I started gaining weight in October 2012, started birth control in December 2012 to regulate & lighten my periods, I stayed on the BC for 3 months(it did lighten my period a LOT but my abdomen kept getting bigger during those months so I decided to stop taking the pill in hopes it was causing the abdominal weight gain). I had also become constipated, laxatives were not helping, when i had bowel movements they were very thin. My abdomen, esp the lower, constantly felt swollen & heavy so I stopped the pill at the end of the 3rd month, the following day I started bleeding a LOT(although my period had ended 4 days prior), I was soaking thru 2 super tampons an hour. This continued for 6 hrs followed by 15 hrs of soaking thru 1 super tampon an hr. A few days later I had my 1st normal size bowel movement in 3 months, 2 wks later I was 8lbs lighter & abdomen was no longer swollen. I assumed the BC was to blame for the swelling & weight gain...4 months later, the weight & swelling in my abdomen started coming back. It has stayed ever since despite diet, exercise, etc.
With my muscles, I'll have days-weeks of feeling ok, having strength, able to do my workout routine with 8 lb weights and then randomly, my muscles will start getting weak again, lasts for a week or so, then goes back to normal & so on.

Hi,
Noted ur reply and understood that your CK was elevated for no obvious reasons and all tests including Muscle biospy were negative. This is very unusual. We ned to find out the reason for high CK. It could be vitamin D deficiency or due to drugs like Statins ( cholesterol pill). It could also be due to myopathy ( a primary condition affecting muscles), but then your muscle biospy returned normal.

Some of the symptoms could be due to Adderall XR as well.
We need to investigate the reason for muscle pain and mucle weakness.

To be XXXXXXX you need to be under the care of a multidisciplinary team with endocrinologist, Physician and a neurologist in the team. You may also need physiotherapy.

Vitamin D levels also need to be checked.

Please google " Cushing's Syndrome" and let me know if your symptoms corresponds to that. If yes, we need to rule out that as well.

Please do the tests which i mentioned earlier. Doing stretch exercises and yoga/meditation might help. You may also benefit from a holistic clinic treatment.

please do not worry. Let me know the reports ( i know that these were done in the past, but you may have to repeat them now.
