What Causes Vomiting Of Blood, Blood In Stool And Nose Bleeds?

Question: I am 30 year old female. The last 3 years I started vomiting blood, sometimes black, sometimes bright red with black bloody stools. I also have telangiactasias on my skin. I get stomach pain only when I have these bleeding episodes. I end up hospitalized at least once a month needing blood transfusions and pain control. Have had countless scopes, surgeries and got tested for many blood disorders. One in particular was HHT. Genetic vascular blood disorder bc my mom also has telangiactasias and gets nose bleeds but my test came back negative but the genetic Dr said he was 99% sure I still have HHT. I was out on estrogen and it's helped a little bit. But now drs are not sure this is what I have bc I don't have the common symptoms of it. Any ideas on what you think it could be? And any more info you need on me just let me know. Thank you. I'm just getting desperate bc it's been 3 years, countless procedures, scans, blood transfusions, etc.... Please any advice will be appreciated!

I wanted to let you know in case you didn't receive my last message I am working on getting all my records for you. Hopefully I can get enough for you before our session closes. Of not I will try again at a later time. But I wanted to make sure I was gonna get the same Dr answering my questions and following up with me. Since I am a difficult case, I would rather work with just one person. So while I am working on getting enough records for you, I had 2 apts since we talked last. One with another surgeon who agrees we needs to remove my stomach if GI and IR have no other options for me. But than I saw my GI Dr who asked me please don't do the surgery until he figures out more on my condition. Bc at this point we still are unsure what blood disorder I have. Still may be HHT but we need to pin point where in my stomach I am exactly bleeding. So the next time I have a bleeding episode I am suppose to page him personally and he will meet me at whichever hospital he is at and scope me again and try another camera pill. He said he will continue to scope me in till he finds the exact bleed/bleeds so if he can find it he can than tell the surgeon exactly where it is so they can remove that portion of my stomach again, hoping to solve the issue. We just don't want to rush into anything bc we still aren't 100% sure what's going on. No one I've seen it spoke with had ever seen a case like mine. They were so stumped they went to extremes and had a nurse sit with me 24 hours a day for 6 straight days while I was bleeding trying to pin point a trigger or figure out if I was really vomiting the blood and not coughing it up. Once they knew it was being vomitied up they continued scopes. My GI has see a duelifoy lesion in my stomach in the past but is not convinced that's where all the bleeding is coming from. Next week I see a new hematology Dr too. I just can't give up and be ok to live like this. So I'm gonna continue to seek out any help from any one! I really hope you are able to look into my case for me once you have the records and give me something, anything....we are just running out of options. Thank you again for your time, it's treating appreciated! Also have you ever had HHT patients and was estrogen the only treatment for symptoms you could offer them?

I can understand your concern. Your case is difficult case and need many follow up question answers. Be sure about that. I will give your all follow up answers and I will help you till you get solution.

HHT is rare disorder affecting 1 in 5000 person and treatment is also different from patient to patient. Estrogen is used as symptomatic treatment to prevent nose bleeding. But it is not the only treatment. Tranexemic acid and other agents can be used in this condition.

Other treatment options depends on site of bleeding and frequency of bleeding.
