What Causes Vertigo, Dizziness, Heaviness In Head And Migraine Attacks?

Question: Hello Doctor,

I had suffered from vertigo two years back. Since then i was on dizzitac tablet for four months bcoz i was feeling dizzy. did ct scan and ear rest like champ and eng. Reports came normal. After six months i became pregnant but from seventh month onwards i was feeling dizzy again and head was getting heavier at back as if something pulling me back. Also left side i was feeling out of balance. Neurologist prescribed me dizzitac for ten days and asked me to wait for pregnancy to get over.

At the time of delivery i got a migraine attack, left side of head blood was russing fully for half an hour. This attack repeadted again after two weeks all of a sudden. I was left with pain at left side which subsised with tablet prescribed by neurologist.

But now whenever i drink some coffee or tea.. orr watch tv. All of sudden i get heavy feeling on upper left side of head. Then evn my left eye starts blinking whenever i try to focus on one point. Sometimes i feel less balanced on my left feet. Neurologist did some exam on me by testing my joints.. she says i am fine ... There is no problem. Also whenever i stand still while cooking or washig.. i begin to move.. i am not able to stand still.
Can u please let me know what could be the problem and what can i do about it.

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

Thanks for providing a detailed medical history.If you would have presented to me as an outpatient with these clinical details,even I would have gone with 'Migraine' as your diagnosis.
The hallmark features to support 'Migraine' in your case are-
1)Episodic headache which have specific trigger factors like drinking tea or coffee and watching tv.
2)Headache is unilateral(on one side of head)
3)Throbbing or pulsatile headache(like blood rushing through the head).

These features are classical for Migraine.It is no XXXXXXX that it worsened with pregnancy, as may happen in many.Dizziness is commonly associated with Migraine.

However,loss of balance and difficulty in walking are slightly atypical for migraine,though may be present.These may also be secondary to dizziness or vertigo.Neurological examination being normal rules out a definite neurological lesion for the same.However,to settle the confusion,a brain imaging like Computerised tomographic scan(CT scan) with contrast study can be done.

Meanwhile,please try to eliminate the trigger factors of tea and coffee from your life.Watch tv for the minimum time possible.

Please discuss with your physician to start you on anti-migraine therapy for acute episodes like ergots or triptans,the one which suits you the best.

I want to know about the off balance feeling which i get on left side. Are there any exercises which i can do for migraine, veritigo or dizziness. If yes it will be helpful if you share.

Also would like to know when i try to focus on something apart from heaviniess in head , my eyes also starts to twich. Can you please let me know about it why it happens.

Thanks for being in follow-up.

Regarding exercises, vestibular rehabilitation therapy can help in maintaining balance.Please consult an expert physiotherapist or a specialist in 'physical and medical rehabilitation' who can give expert teaching on vestibular rehabilitation exercises.

Focussing something,makes the eyes twitch actually indicates,this problem being related to anxiety.These may be benign fasciculations triggered by anxiety.Please start doing mental relaxation exercises/meditation/yoga and this problem should resolve.
