What Causes Variations In Fasting Glucose And HbA1C Levels?

default
Posted on Thu, 30 Jul 2015
Question: Hello. I am interested to know what it means when on April 30 I had fasting glucose of 121 and A1C of 6.5, and on XXXXXXX 20 just two months later a number of 86 and A1C of 6.3. Triglycerides and fructosamine in normal ranges.

Also why would creatinine in urine be 13.6 but creatinine serum be in normal range?
Thank you.

Brief Answer:
Normal urine creatinine in men is 14 to 25 mg per kg of body mass per day

Detailed Answer:
Hi XXXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Your fasting glucose and HbA1C levels are in normal range in the report dated XXXXXXX 20.

2. The normal urine creatinine is expressed as mg per kg of body mass per day for humans. The normal range of urine creatinine is
(i) 14 to 25 mg per kg of body mass per day for men
(ii) 11 to 20 mg per kg of body mass per day for women

These values might differ slightly with the testing laboratory

3. If your urine creatinine is 13.6 (mg/kg/day) then it is normal. Creatinine is excreted in urine and therefore the normal range of creatinine in urine is higher than in blood. Serum creatinine measures the creatinine level in blood.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Thank you. The lab considered the creatinine low at 13.6, with normal at 27 in urine. How do I understand the fluctuations between the April and XXXXXXX tests in glucose with A1C when labs have ranges for glucose with maximum of 100 and A1C at 6.0?
Also I had a PSA of 0.40 in April, but in XXXXXXX I had the tests when I had a UTI.
That test showed a total PSA of 7.63 and Free PSA of 2.570. Does that increase from April signify an effect of the UTI? My PSA historically is always under 1.0.
Thanks.

Brief Answer:
Please have your PSA tested after a month to get an accurate value

Detailed Answer:
Hi XXXXX,
Thanks for writing back with an update.

1. The creatinine in your urine is not a serious concern and the report is acceptable.

2. Regarding the fluctuations of HbA1C, it is important to keep in mind that the blood glucose level is a dynamic variable and is not constant over time. The fluctuation is mild in your case and might be attributed to the comprehensive intake of sugar containing substance over the interval between April and XXXXXXX The value of 6.5 was slightly high and 6.3 is in upper normal limit. The range of HbA1C from 5.7 to 6.4 indicates a high normal level.

3. PSA is often elevated due to bladder and urinary tract infections. I suggest you repeat PSA once again after a month after you are cured of the UTI. This will give a true value of PSA and is important to be recorded.

Hope your query is answered.
Do write back if you have any doubts.
