Question: I saw an endodontist today, I have an abscess in an old root/canal crown in an upper #3 molar....I have been on clindamycin for 36 hrs....I am having unbearable, excruciating pain, taking motrin 400 q2h....just barely able to stand it...thoughts? It is 2:15AM. XXXXXXX (I am a surgeon)

There can three reasons for this pain and dizziness
1-first it can be possible your infection of tooth is causing this problem, as tooth pain is a severe pain and even it is radiating in nature and this motrin is not able to control it, so in this case you should start with a dispersible paracetamol, and visit your dentist the next day.

2-second it can be possible that this infection of third molar has involved the sinus as it is very close to the floor of sinus, to get it confirmed you have to get an x ray done of that region which will show you the infection.

3- third it can be possible that while the treatment the dentist might have perforated to soft tissue below the tooth.

Right now for relief you should first start with a dispersible paracetamol or other dispersible analgesic. And then visit your dentist and get an x - ray done of that area which will show the infection.
