Question: Hi Doctor, I am giving a detailed account of my case herebelow. Kindly help me. LIST OF EXISTING SYMPTOMS (CURRENT) - Tightness of Neck in the Nape Region. Muscle behind the neck just above the shoulder seems swollen (more while working on laptop and less when sleeping) - Unable to walk with ease (shoes feel slightly heavy). This is especially predominant after lunch and the most worrying symptom. - Feeling of Light headedness while walking (more predominat after lunch) - Stabbing Pains which come and go in the neck quite often (more while walking and working on laptop). But similar pains in shoulder, elbows and back of knees come irregularly once in 2-3 days. - Slight feeling of lump in the throat. - Feeling of slight dryness in the Throat ( not Thirst) especially in the morning. - Slight TINNITUS in the left ear ( low level and plain howling sound) almost continuous except for while I am talking - Some quick pain (drilling pain) just behind the sternum ( once in a while). Pain comes and goes away very fast .. may be in around 2 minutes. This might happen for 4-5 times once in every 2-3 days. - Numbness on the top of head ( scalp) sometimes while in sleep. Other Important Factors : - The above symptoms are existing for the last 2 months. - I have completely quit smoking for the last 2 months.(Was smoking10 -15 cigs a day) - I have been away from Liqour also for the last 2 months. - I used to take a Glass of Water with Honey, Lemon and one lobe of Garlic in the morning every day for almost a month. But I stopped the same from a week back - I have a history of a small SYNCOPE event ( for about 30 sec ) in 2008. - I have started a new drug ROSULIP 20 mg ONE month back. (I am already taking Ecosprin-75mg once a day for the last 6 months) - Visited 1 Gastroenterologist, 2 Spine Surgeons (at Apollo & Care), 4 Genl Physicians, One Endocrinologist, One Ayurveda Doctor so far. My own Assessment : My own study/reading from the Net makes me feel that the following should be ruled out : 1) Primary Biliary Cirrhosis 2) Eosophagal Abnormalites (Yeast Infection/ Inflammation or Cancer) 3) Infection of spine due to syphilis ? ( May be 3rd stage ?) 4) Severe Gastric Inflammation /Gastritis / Hyper acidity due to H Pylori infection 5) Any Lung Disorder (emphysema ?) 6) Any Tumour OR Aneurysm in the Brain/Nervous system ?

Brief Answer: You need blood tests and various scans. Detailed Answer: Hi, I have gone through your query. Most of your symptoms seems to be due to compression of your nerves in neck region. For this you need to get x-ray cervical spine done in AP and Lateral view and blood tests including fasting blood sugar, E.S.R, C.R.P, RA factor, serum uric acid, E.C.G, SGOT , SGPT and discus the reports with your doctor. Your doctor may do your physical examination and advise you MRI scan of cervical region to see if needed to see the condition of inter-vertebral disc, cervical nerves and surrounding soft tissues. He may also advise you CT scan and spirometry of lung to know the condition of your lungs. It is good that you have quit smoking and avoiding alcohol. One thing you must know that drug ROSULIP you are taking may also cause muscle pains so you must consult this with your doctor so that he can change it or alter its dosage. THanks for the response doctor. I have attached the entire record of investigations inlcuding all those suggested by you. All these reports have been seen by local XXXXXXX doctors also but they are of the opinion that it is simple spondylosis. My problem is that I am not much bothered about the pain. It is the giddiness and the strangling type of sensation in the neck that is bothering me the most. Sometimes I feel someone is choking me. I will stop Rosulip forthwith as I can see some relation on the same and get back to my doctor for alternate medicine. DO you feel that the spine is not really pressing against the spinal canal. Can I be sure that I will not create any emergency with this problem at home. Would you advise any neurologists consultation for me ? Also in case it is only spondylosis what could be the best line of treatment to be followed ? Also would be great if you can pl advise of any institute in Ahmedabad where I can seek local help for spondylosis.

Brief Answer: You can consult neurologist. Detailed Answer: Hi, Thanks for the query again. You have increase uric acid levels so you need treatment to decrease uric acid level upto normal level. You may consult orthopedic surgeon for that as there are number of drugs to decrease uric acid for this, your treating doctor would advise you the safest drug for you after doing your physical examination. You have compression of cervical nerves, you must discus with your doctor regarding aceclofenac+thiocolchicoside combination, cinnarizine, gaba-pentin, vitamin b12 etc. Once pain and giddiness subsides your doctor may advise you static cervical spine exercises as uploaded on you tube. You can consult neurologist for all that. He may advise you EMG and nerve conduction velocity test to examine nerve root function. You should try to avoid sitting in front of laptop or PC as much as you can. Apply hot packs for 30 mins in morning and evening. For spondylosis best line of treatment depend upon individual patient e.g aceclofenac+thiocolchicoside combination, naproxen, antidepressants and at last surgery. So it is the treating doctor who after doing physical examination and blood tests tally all the reports and advise best medication for the patients. I will advise to consult doctors in institutes like PGI Chandigarh or AIIMS instead of Ahmadabad if you want to get another advise for your symptoms. I hope you have got your answer. Thanks.
