What Causes Throbbing Headache And Severe Chest Pain?

Question: I woke up not feeling well & had cloudy horrible smelling urine. Now it has little specs of blood in it. I also had severe left sided chest pain last night, the pain was in my back and stomach had a weak threads pulse, but settled out. Was going to go to ER but was too weak. Also really bad pounding headache. How much does this cost?
I live in Canada & do not have pay pal or anything as I live in Canada and have almost zero credit as in consigned a loan for someone who died...long story. Aw I will set up a pay pal account today or go to ER due to my persistent throbbing headache

Welcome and thank you for asking on HCM!

I would explain that the throbbing headache may be a migraine attack, exacerbated probably by an infection.

Have you measured your blood pressure during last night? Blood pressure changes (high or too low) could also lead to pounding headache, weakness and weak pulse.

What about your body temperature? Do you have fever?

I would advise you to closely monitor your blood pressure and body temperature and refer these values to your doctor.

The smelly urine with blood in it indicates an urinary tract infection.
The pain in the back and stomach may also be related to this infection.

I would exclude any cardiac cause related to this clinical scenario.

Coming to this point, I recommend consulting with your attending physician for a careful physical examination, and some tests:
-chest X ray study
-lab blood tests (complete blood count, PCR, sedimentation rate, kidney and liver function tests, blood electrolytes)
-urine analysis, coupled with urine bacterial culture for possible infection.

An abdominal ultrasound of your urinary tract may be needed.

Topamax is a really effective drug against migraine (as preventive therapy) and sleep problems. But it has been associated to urinary tract stones as an adverse effect. So an ultrasound would help exclude this possibility.

Regarding gabapentine, I would not recommend it for migraine, as it is not a first drug of choice in this disorder (topiramate, lamotrigine, betablockers, valproate, antidepressants, calcium channel blockers are more preferred in migraine than gabapentine).

I don't think it is a UTI as I have had a few before and have no other symptoms.
Can u take calcium channel blockers with Tipiromate?
Also, I forgot to mention I am taking 20 mg of Cipralex daily

Did u say I should rule out cardiac issues? As I am still having some mild Chest pain?

You are right when saying that most of UTI have no other symptoms (this is more common in lower urinary tract infections).

But the smell and presence of blood in your urine is very indicative of Urinary tract endothelium damage and possible infection.

That is why I would highly recommend performing an ultrasound and urine analysis coupled with urine bacterial culture.

Your symptoms don't seem to be related to any cardiac disorder.

The chest pain may be related to inflammation. Some blood tests as I mentioned before (including inflammation tests) are needed to exclude this possibility.

Regarding concomitant use of topiramate and calcium channel blocker (like verapamil, diltiazem, etc.) I would not recommend this combination.

Topiramate is known to induce their hepatic metabolization the concomitant use with them could increase the risk for adverse effects and reduce their effect.

Actually, I'll go to my Dr. & see if I can get these tests done. The headache has been lingering for months. They wanted me to take 1600 mg of gabapentin which is ridiculous. I take 300 mg.

The chest pains scared me last night. They were severe.

Dear Dr.
I believe you are correct in your diagnosis as my urine still smells bad & I did have an episode of a fair bit of blood in my urine about 6 weeks ago but was unable to get it checked out due to work. It went away on its own...that time I did have symptoms of a UTI.

I have an appointment with my family Doctor on Monday.

I doubt she will do all the tests you have recommended, though I agree they should all be done. Thank you again.
