Question: My father(73) had knee pain after an object fallen to his thighs. Dr suggested to go for knee surgery after seeing MRI. I wanted to seek advice here.

Impression (from MRI)
a. Thickening and altered signal intensity of anterior and posterior cruciate ligament - suggestive of interstitial tear.

b. Osteopathic breaking at tibial and femoral condyle with subchondral cysts in the tibial and femoral condyle-suggestive of osteoarthric changes
c. Altered marrow signal intensity in the lateral femoral condyle and head of the fibula- suggestion of bone contusion
d. Joint effusion with extension into medial, lateral and supra patellar recess.
e. EDEMA adjacent to medial collateral ligament - grade 1-II sprain

Your father has suffered a grade I-II sprain and a partial thickness tear of the cruciate ligaments. Since there is no meniscus tear the arthroscopic surgery can be avoided if he can give adequate rest to the knee joint with the help of brace for at least 6 weeks. If he needs an early restoration of activity then a surgical repair is recommended. The bone contusion and edema are due to the blunt injury and will subside in few weeks with anti inflammatory medicines.
To avoid the osteoarthritic changes to progress further after the injury he needs regular physiotherapy sessions after the injury heals and joint lubricant supplements like Glucosamine after consulting his orthopedician.

How complex is the surgery? My Father is 73, what are the complexities may arise if he performs the surgery.

The surgery is not very complex. In experienced hands it is a simple case. If your father's cardiac status is normal then any anaesthesia related complications are not likely. Healing is fast. Post operative recovery is usually smooth if there is no infection. He will need regular physiotherapy after the surgery for a complete recovery and to restore full range of movement.
