What Causes Testicular Pain With Swelling And Redness Of The Testicle?

Question: Last night at approximately 11:30pm I began to feel some lower back/kidney pain that felt a little like gas. I was masturbating and when I ejaculated, it was very painful. Once I finished I immediately experienced severe pain in my right pelvis and right testicle. The pain was throbbing and burning and my testicle was very sensitive to the touch and hurt a lot. This pain lasted for about 2 hours before fully subsiding. I have only had 1 sexual partner in the past 2 years and we have had unprotected sex. We are dating so we have trust. I have not experienced any further pain so far but I have noticed that my right testicle has some redness but no swelling. At times it is higher then the left and sometimes lower but it no longer hurts. Could I have some testicular torsion that only lasted a little bit or could this be result of an std or other type of infection?

You recently had severe testicular pain with mils swelling and redness of the testicle. This sounds very much like epididymitis. The testicle has small tubules that transport the sperm from the testicle to the seminiferous tubules that store the sperm. these tubules make up the epididymis. Occasionally for really unknown reason the epididymis can get infection fro unknown reasons. It causes pain that radiates into testicle. You may need antibiotics to help reduce the pain.

A torsion testicle would not recover spontaneously. These are medical emergencies and require surgery to fix. So I do not feel you have a torsioned testicle

Thanks doctor. I had another experience except no pain at all during or after ejaculation. Is epididymitis a serious problem? What can I do in the mean time to treat the condition until i can make it to the doctor?

Epididymitis is a very common problem and is usually self limiting. This means it usually gets better on its own. Sometimes though if the pain persists then you may need treatment with antibiotics. Since your symptoms are not persistent and only intermittent I would wait and see. In the meantime while you are waiting, wearing a good supportive underwear will help reduce the pain.

Hi doctor, i made a visit to the walk in clinic today. The DR there said that something like Epididymitis would cause constant pain, is this true? He recommended me for an ultrasound of the area. I have felt no further symptoms except for some slight discomfort when i wear slacks. I have noticed that my scrotum is red but not swollen and my right testicle lays more horizontal than the left. The only other thing i can think of is that i did stress my body doing lots of shovelling over the last few days before the pain started. Could that have caused a hernia issue resulting in the pain?

Yes Muscle strain could also cause the pain

Yes the pain could be a result of stress on the lower inguinal muscles from shoveling. You could have also stressed an inguinal hernia but if it were a hernia the pain would be more constant and associated with a bulge in the inguinal area. I think the doctor is recommending the ultrasound to determine what maybe causing the pain. On an ultrasound they can see if there is any torsion, hernia varicocele or inflammation of the testicle. I agree with proceeding with the ultrasound. In the mean time wear supportive underwear and use Tylenol or ibuprofen for the pain.

2. Rule out inguinal hernia, varicocele, torsion or inflammation
3. Wear supportive underwear
4. Tylenol or ibuprofen for pain
