Question: TWO FRI.S AGO I HAD PAIN AND. DIFFICULTY URINATING WITH VERY WEAK STREAM AND A LITTLE BLOOD FOLLOWING URINATION. THIS CONTINUED ALL WEEKEND UNTILL SUNDAY EVENING WHEN I PASSED WHAT I BELIEVE WAS A KIDNEY STONE. THIS RESULTED IN NO MORE PAIN OR DISCOMFORT DURING URINATION AND MY STREAM RETURNED TO NORMAL . A LITTLE BLOOD WAS PRESENT IMMEDIATELY FOLLOWING URINATION BUT STOPPED AFTER 2 MORE URINATIONS. ALL SEEMED OK UNTIL TUESDAY AFTERNOON MY BELLY BUTTON WAS VERY TENDER TO THE TOUCH AND SWOLLEN WITH A 1/2 INCH LUMP DIRECTLY UNDER NAVAL BENEATH THE SKIN. THE FOLLOWING DAY THE AREA AROUND THE LUMP BECAME INFLAMED AND REDDNESS TO SKIN APPEARED AND VERY TENDER TO THE TOUCH. AS THE DAY WENT ON THE INFLAMATION AND LUMP GREW DRAMATICLY. I WENT TO AN URGENT CARE FACILITY AND WAS PRESCRIBED AN ANTI-BIOTIC WHICH I AM STILL TAKING. THE INFLAMATION IS GONE AFTER 8 DAYS OF THE MEDS., BUT THERE IS STILL A LUMP UNDER NAVAL. THE NAVAL IS STILL SLIGHTLY SWOLLEN, BUT NO LONGER TENDER TO TOUCH. EARLIER THIS EVENING I BEGAN FEELING SLIGHT DISCOMFORT RIGHT SIDE OF STOMAC BELOW NAVAL. IT IS NOT PAINFUL, BUT I AM CONCERNED, IN THAT NAVAL IS SWOLLEN, LUMP STILL PRESENT AND THIS NEW SYMPTOM. I HAVE HAD A MILD CASE OF GOUT FOR ABOUT TEN YEARS. THE FLARE UPS ARE FEW AND FAR BETWEEN AND NOT THAT PAINFUL, ALTERNATELY AFFECTING EITHER JOINT OF BOTH BIG TOES. I HAVE MEDS TO CONTROL THE URIC ACID. THREE WEEKS AGO I STARTING TAKING A TIME RELEASED PROTEIN SUPPLEMENT WITH MY MORNING BREAKFAST DRINK. WITHIN A FEW DAYS I BEGAN HAVING FLARE UPS OF THE GOUT AND THEN THE PASSING OF THE KIDNEY STONE. I IMMEDIATELY STOPPED THE PROTEIN. GOUT FLARE UPS CEASED AND I ONLY PASSED THAT ONE STONE. COULD THESE SYMPTONS ALL BE RELATED TO KIDNEY STONES?? I HAVE BEEN TRAVELING OUT OF TOWN FOR THE PAST FEW WEEKS WHEN ALL THIS BEGAN. I WILL BE HOME IN A FEW HOURS. SHOULD I GO TO EMERGENCY ROOM OR WAIT TO SEE MY DR. ON MONDAY? AS OF NOW, I AM NOT EXPERIENCING ANY PAIN. ONLY MINOR DISCOMFORT IN LOWER RIGHT ABDOMEN.

Hello sir,
I have read your case history and it looks to me that frequent travelling and protein drink has flared up your GOUT. Lump which you are having at naval can be a tophi. It is uric acid deposition and common around joint but can happen at such positions also. I think in the ER you have been given strong pain killers and antibiotics and that has caused gastritis in you leading to abdominal pain.
At present If pain in abdomen is not much then you can safely wait till Monday. I have following recommendation for you.
1. Avoid chicken or egg, also avoid alcohol. All these can trigger Gout episode.
2. Do not take any medicine empty stomach.
3. If you would be my patient then i would have suggested you Pantaprazole tablet twice daily to take care of gastritis.
4. Do not take any spicy meal.
5. Keep a close watch on abdominal pain. If you feel that pain is increasing and you are not feeling well, abdomen is getting tense then you need to rush to ER.
If none of this is occurring you can safely wait till Monday.
