Question: Dear Doctor,

In 2009 I got Diagnosed with IBS .....

Now in 2017 here is what is going on ....

Pain in Suprasternal Notch ....... Bottom of Suprasternal Notch ..... And a Lot of Tickling in that area as well

I thought I was suffering from GERD because I had all the symptoms ...... So I went to the to the Doctor for a Ton of Tests .....

2 Full Blown Blood Tests CBC .... CMP .... Thyroid Tests ..... Everything is Extremely Perfect with ALL Blood Tests ...

Chest Xray - Normal ...

Ultra Sound Entire Neck and from the Bottom of my Rib Cage ALL the Way up to my Chin -- Normal ....

ENT -- Did Throat Exam ..... Said I had Very Mild Silent Reflux .....

Wore a 24 hour Heart Holter -- Normal

I had ALL the Symptoms of GERD .....

Stomach Pain -- ALL over in the stomach and then also in Designated Area's

Bloating at times

Lots of Noises coming from the stomach ... Gurgling and like a half bottle of water swishing around...

Lots of Gas .....

Sometimes Light Acid Reflux ... Not often but I did have a little ....

Pain and Bloating after Eating .....

Upper Spasms in my Suprasternal Notch area

Feeling like Any time I was going to burp and feel like I was going to burp up Acid Reflux ..... But never did .... Just felt like it was hanging around below my Suprasternal Notch and at anytime it was going to happen ...

Hard Long Dry Burps .... When I would burp .... The pain in the Suprasternal Notch would subside ..... Get Less and less ....

Pain in Suprasternal Notch -- Like Lumpy Feeling -- Like there is a Golf Ball in my Throat .....

Trouble Swallowing Food .... Sometime have to turn my head to the right and swallow hard to push food down due to feeling dry ....

Excess Phlegm ....

Feeling like I have Excess Saliva .....

When I would swallow liquids SOMETIMES it would feel like the Liquid would be at the back of my throat and go down slowly ...... Like a Funnel.....

Sometimes during the Day.... Every now and then I will feel Absolutely wiped out .... Fatigued and Flu Like and within 20 minutes it ALL goes away .....

My OCD and Anxiety would make ALL this feel worse .....

So I seen a Functional Doctor and did his Protocol .....

Changed Diet

Supplements ...

Even did a 14 Day Juice Diet ..... Lost 15 lbs ... I didn't need to lose ....

Included Apple Cider Vinegar - 2 Tablespoons and Water every Morning .....

So Most of ALL the Symptoms went away ......

Then I was having Pain in my Chest .... Mainly on the Right side of my Breast Bone ....

Doctor said it is Costocondritis ...... When I would push on it hard enough I thought I could feel Sensation in my Suprasternal Notch .....

Plus I am High OCD and Anxiety -- HIGH ... Doctors are telling me that Anxiety is Mimicking Symptoms .....

I am at a Loss .....

So Now I am back with Functional Doctor -- Regular Doctor says I should follow his instructions for 3 months ..... because I Only followed them for a month and then switched to 3 different protocols and tried to combine a few as well .... Not a Good Decision ....

My Regular Doctor said to Stop Everything and go back to Functional Doctor and Follow Protocol for 3 months and give it time to work .....

I just started doing Coffee Enemas as well --- This is wiping me out .... Ugggggh .... Two was Enough ... I stopped doing that .....

I am ALL messed up with what to do ......

Thanks for the query

I suggest you get an ultrasound abdomen to rule out Cholecystitis which can cause similar symptoms.

Also do an upper GI Endoscopy with Helicobacter pylori urease testing to confirm GERD and infection which can cause these symptoms.

You also need treatment for your anxiety concomitantly.

Ok ....

So I got this Pain by the right side of the Breast Bone and the Doc says Costochondritis ...... Now I am getting a Pain on that is Dull at times in My right Shoulder Blade but closer to the middle of the back ......

Then I keep poking at my throat .... and the Doctor says --- You throat should not be pushed on and poked around like that ....

At times when the back of the Right should blade .. closer to the middle of trunk happens ... It seems like at times pain will radiate up the neck and give me a sore side of the neck as well .....

All my Tests above were clear ....

Now they think my Mind is creating pains and symptoms ....

Ok .... Let me see what you think about this all going on .....

1. So Doctor says Costochondritis is the cause for the pain on just the right side of my chest/breastbone and the same height in my Right shoulder blade ....

2. I get Lower back pain as well ..... right side ... belt line area ..... And maybe that is from sitting/slouching a lot ..... Posture .....

I ate Salmon and a Baked Potato tonight and I had to swallow a few time hard as food felt stuck in the back of my throat ( bottom) .... :(

3. Now my throat feels tight ..... Not only the throat but all the way back to the muscle on the back of the neck .... Sometimes I even get tingles on my head from it ..... Maybe stress..... I am quite OCD/Anxiety .... And maybe Anxiety is making ALL these symptoms themselves ..... Dont know ???

4. I also get a bad Neck pain on the right side of my neck.... on the main muscle that will hurt all the way up and on the back of my head -- that main muscle ......

5. Doctor says I am pushing ... poking ... grabbing...squeezing and doing too much messing around with my throat ....He says too much in there and the throat doesn't like to be irritated like that .... and that can take a long time to heal .....

6. All the back pain Shoulder .... Lower Back .... Chest --- just a little to the right of the Breast Bone and Neck pain Muscle and behind ear ... All this is taking place on my Right side .....

All these symptoms could be explained by low Vitamin D. Please do get a Serum 25 OH Vitamin D analysis. Especially the muscle pains. Neck pain and back pain can be due to spondylosis.

Throat congestion can lead to the similar throat symptoms as you've mentioned. You can try betadine or salt water warm gargles.

So I go to a Very Well known ENT here in the States and I finally breakdown and get the Scope that goes into the Nose and Down my throat ......

They tell me You are Very Healthy and even sees the Food tubes that transfer into the Esophagus and says .....

No Silent Reflux ..... No Nodes on the Vocal Chords ....

Everything Looks Very Healthy .....

If your mouth feels dry ... it is because it does look a little dry .... So drink a little more water when you Eat ......

I have Post Nasal Drip which causes the Tickle Feeling and Itchy feeling in my Suprasternal Notch and will cause a cough with some phlegm when it tickles .....

Thinks the Twitch by my Thyroid is just Anxiety .....

But says ...... Very Healthy No Need to do Anything ..... Don't think about things as Stress will make you think there is something somewhere and there is not ..... Phantom symptoms can happen anywhere in the Body ... The Mind is Powerful ....

These are symptoms of GERD which can be aggravated by anxiety.

Answers to your questions

1. and 2. - If you burp before or after hiccup there isn't any specific pathology or cause. It indicates diaphragmatic irritation due to stomach and oesophageal distention.

3. Daily morning hiccups could suggest diaphragmatic irritation and needs to be investigated. Oesophageal manometry will be useful.

4. The frizzy bubbly feeling is due to acidity. You are experiencing hyperawareness due to anxiety.

5. Anxiety and OCD can aggravate GERD symptoms as well as oesophageal motility disorders.

It is advisable to avoid looking looking up information on the net because most of them are unstructured and can lead one to uncharted territory without a clear understanding. Says .... You could have GERD but ..... All indications of being anything else ..... There are No Indications to get Scoped .....

So I wake up and my stomach noises are loud ......

Now sometimes I have been waking up with pain in the stomach around belly button or lower ..... I do always have bowel movements a little after I awake ......

But I am concerned abut the growling and noises ...... and my OCD is going through the roof......

Hiccup usually occurs due to diaphragmatic irritation. Burp indicates letting out of stomach digestive gases.

When the gas travels up the oesophagus it can stimulate an Irritable diaphragm and cause Hiccup followed by Burp.

Burp followed by Hiccup can indicate a larger gas bolus being propelled up thus stimulating an Irritable diaphragm.

These aren't signs of cancer.

Ok .... Here's one for you .....

1. I get a pain dull sharp pain on my right side --- Like in the Middle of my shoulder blade and sometimes it will be close to the spine ....

I see a Rolfer and he says its always tight back there .... and he seems to always say its ribs ....

Sometimes I get the pain in the front of my chest and it will radiate to the back right upper shoulder blade part ...

It's the kind of paid where you want to back up against a wall and just move from side to side .....

All the Docs I see seem to tell me its the Costochondritis .....

I do lift ( work out ) heavy 5 times a week .... and I GOLF A LOT .... And am always practicing .... a lot of twisting ....

The Pain seems to come and go ... I will admit ... I do sit a lot and don't have the best Posture ... I do slouch quite a bit .....

A lot of time it seems to be linked to the right side front of my neck ..... And sometimes when I stretch my neck to the left side ... I can feel the pain in the Chest or the Back or both .....

2. And my Ribs are Tender to touch a lot of time on the right side .... just under my nipple in the front of my chest .... The Two lower ribs ..... and again .... I when I touch it It is tender .... I can always touch and recreate all pains where they are happening ... most of the time ....

Even though ALL my Tests came out Perfect ....

I still get twitches around my thyroid .....

My Costochondritis flares up as well.....

As you know My Anxiety kicks in when things are happening ....

My throat will hurt from time to time where I was poking on it ALL them time .....

The side of my neck will hurt and it will shoot a pinch into the right side of my chest ..... If I push the area of my neck where it hurts ... Not only will it hurt in my neck ... but it will make the place in my chest hurt along with the back of my Shoulder Blade .... Is this All Connected ....

Should I be concerned ..... I am scared with ALL this .....

Posture issues can cause these problems which could be associated with the gastritis and your anxiety.

The pain and difficulties you feel have not been quantified by all the necessary Medical tests so it could be functional with a psychological component.

Please do an MRI scan of the neck and oesophageal manometry test to rule out motility disorders and inflammation.

I did an Ultra sound of the Neck and Many Many Neck Exams .... And All the Docs say I am Fabulous ....

I cannot do a MRI due to the Tinnitus Damage in My Ears ..... The Knocking .... Uggggh .....

Plus .... When the ENT pulled the scope out of my Nose the first exam .... I got injured ... I don't think I can endure a 2nd Anything getting put up my Nose until I am Completely Healed .....

Alright ...... Another Question ......

So I suppose to have this Chostochondritis ......

And it feels like sometimes it my be in the front of my chest and radiates to the same spot on my back in the Upper part of my right shoulder blade .....

Then sometimes it will hurt Right side Rib Cage .... Under Nipple ... but in the ribs .....

Then it will at times be in the Same Spot ... Left side Rib Cage Under the Nipple ....

All this along the bones ..... And I can Press where it hurts and it will hurt there .....

Then it All goes away ......

So I seen a Chiropractor that tells me I have a rib out in my back .... That is why it hurts back there

The Rolfer I see says I have ribs out as well .....

Then it hurts in the front of my chest near breast bone ..... Same location but in the front as well .....

Then it feels like there is a Nerve in the side of my neck that is all connected to this ....

So at times it will hurt in all 3 places ... When I swallow ... sometimes ...

When I take Deep breathes sometimes or when I cough ....

It just seems like all this is connected ......

It is also hard to breathe at times with the ribs being out ......

Once I get an adjustment or the Rolfer works on them ......

It seems the berating is better and I don't have issues ......

But if I work our or Play golf as I do ..... Twisting and Pounding the ball as hard as I can at 58 being in GREAT Shape ..... After that I feel it ...... And I need body work done .... I golf about 4-5 times a week .......

It's Brutal ......

Here is something that is also a flare up .....

Every morning when I awake .... I do sit ups .... and the Costochondritis ..... if that is what it is .... In the front of my chest ...... I can feel a little ache while I am doing the sit ups .... whether I have my hands behind my head or not .....

Why I think most of this pain is skeletal and muscular is that ..... When I move certain ways is when I can feel soreness ..... or irritation ......

I do NOT do a lot of stretching .... I know .... Not too good ..... Especially lifting Heavy weights in the gym and Playing so much golf .....

So when I do reach my right arm over my head to stretch .... I can feel the sensation of the pain ..... and when I move certain ways I can feel these issues .....

I think if I had soft tissue damage ..... This would not be .... and the pain isn't always constant ...... Which leads me to the theory of Skeletal and Muscular Irritation ....

Along with irritation in neck just above my collar bone .... Half way between the chin and the end of the jaw ... but on the bottom of the neck just above the collar bone ... Like a nerve or something .....

What are your thoughts .....

Another thing I notice ....

I am working out today ( back ) and when I do rows and back exercises .....

When I get to the full potential of the pull or lift .... It will hurt in that area .....

That is what leads me to believe ... what is going in is Skeletal and Muscular ....
