What Causes Stinging Pain During Urination And Pus Like Material In Urine?

Question: Further question after visit by emergency doctor at 4.30 am this morning.
Time is now 10.35 am. on Friday 21/8/15
( refer to history -catheter removal 20/8/15)
At midnight last night
I called emergency service because:
Building from 8 pm I was not only getting stinging pain when passing urine, but, over the four hours to midnight ,the pain (scale 1-5)
increased from 1 to 5 all the time
with 10 when passing urine!
Emergency doctor was arranged who visited as above.
At 11.45 pm ( quarter of an hour before calling emergency) I took a quarter of a teaspoon of bicarbonate of soda in water.
When my phone call to emergency finished at 12.15 am ( half an hour later) the persistent pain ( between passing urine) subsided. So I continued the dose every half an hour.
This has meant that I only get stinging ( still bad) when passing urine. I understand this is to be expected for a day or two.
MAIN REASON I am asking this question is that I was not warned to expect:
After passing a little urine at 6 am the flow became a total thick liquid (pale coloured pus - no stinging sensation while it passed at all)
A few hours later ( around 10 am) Two urine flows included slightly pink pus included with urine.
The earlier infection appeared to have been dealt with by the course of Trimethoprim antibiotic.
Is this pus the residue of that previous infection ?- to an amateur like me it appears to be so, but no nurse or doctor led me to expect such a quantity of pus.
Other than this, after just over 24 hours, urine flow is controlled about every 15 minutes seems to suit. I don't get a strong urge or pain before, but only stinging during passing urine.
I hope that progress will continue- I've never been through anything like this in 80 years!
Is the pus just normal?
I have saved it- should I show it to my doctor?

I have NOT started to use the second course of antibiotic, as I feel that this pus is the residue from the earlier infection. Your advice on this will be welcome.
Things seem to be under control.

1. The pus-like material appears to be urine sediments. It is expected sometimes in early morning sample (as it is a concentrated urine sample and also due to overnight settling of the urine).
2. The stinging is expected as mild urethral damage could have occurred during catheter insertion and removal.
3. Not to worry about the pus-like material, it is only urine sediments - containing protein and cellular casts.

As things are under control, do continue the same.

Thank you you for the explanation.
I do have another two questions, please.
1. After catheter removal I was told that a problem that was to be expected was that urine was likely to be retained in the bladder, even though I was regularly attempting to empty it. I know this as an apparent problem with many elderly people.
However, when I saw, what you now have explained is sediment, It made me optimistic that I had, at that time, perhaps almost drained the bladder on that occasion. Am I being too optimistic?

2. This question relates to the hernia operation that I had almost 3 weeks ago. In hospital very soon afterwards I noticed that my LEFT testicle was about the size of a pear (this was the side of the, possibly larger, hernia, the only one that I had reported) The other testicle did, over a few days enlarge only slightly. The other bruising that followed the operation, looking like a blue/black pair of small swimming trunks, has disappeared now, but the left testicle has only reduced to about 2 inches, double its usual size. After three weeks is this normal progress or something to mention to my doctor?

It will be helpful to receive your answers to these two further questions.

Thanks for asking again.

Based on further queries, my opinion is as follows:

1. Urine retention due to prostate problem is common in elderly. Maybe, on that ocassion, excess sediments present, could have come out. However, I am not optimistic, that at this age, you were able to completely drain the bladder. Improving hydration ( at least 3-4 liters of water per day) will reduce sediment formation due to dilution.

2. Not to worry about left testicle. Only if its increasing in size, its worrisome. Otherwise, it will slowly reduce. Possibly there is fluid accumulation around the testis and it could take another 2-3 weeks to reduce. However, if there is increasing in size, it will be worrisome.
