What Causes Spreading Flat Purple Spots In Thigh,groin And Buttocks?

Question: I woke up this morning to find a small flat purple spot on my inner thigh by my groin area, by the time the afternoon hit it had spread and i now have more spots on my inner thighs and buttocks, what could this be? I have never had this before and the only change to my regimen is champix

Welcome and thanks for posting you query to the forum.

I can understand your concern for spreading flat purple spots in your thigh and groin and buttocks area.

After reading your query with diligence I can make out a probable diagnosis of "Purpura" or commonly known as Ecchymotic spots. It is due to extravasation of blood under the skin which gives purple spots due to deoxygenated blood.

In most of the cases it is a normal phenomenon and subsides by its own but in some cases can be a part of internal problem. Therefore to confirm my diagnosis and rule out any other cause kindly let me know whether you feel any pain or tenderness over these areas, any other medication consumed like pain killer tablets or related thing and last menstrual cycle.

There is no pain or tenderness at all and no irritation. I am on pain medication from a previous back injury. I take oxycontin 5mg for the pain. I do also take vimovo which is an anti inflamatory.
I also just finished my last menstrual cycle on Monday of last week
Is there any form of treatment for this? I have been taking pain medication for approximately 2 years to treat my chronic back pain and have never experienced this type of result before. It does concern me to some extent since it is abnormal to me

Welcome again and thanks for posting important details.

After geting mos tof the imformations let me guide you best for your condition. Regarding the problem of Purpura which we call medially as "purpuric or ecchymotic spots" is common condition seen due to multiple causes.

In your case it can be a normal phenomenon or can be due to other causes.
As guided earlier purpura is simply formed due to extravasation and collection of red blood cells from the vessels under the skin. This results in formation of reddish spots which we call as Bruise in simple language.

There are multiple of factors causing extravasation of red blood cells from the vessels-

1. Due to trauma to the small blood vessels of the skin.

2. Due to some problem in the platelets ( the cells which prevents the extravasation of red blood cells from the vessels and forms the clot).

3. Due to problem with the coagulation factors which helps in repair of the injury to blood vessels and helps in clot formation.

4. Due to combination of all these factors.

So you can probably get simply the factors involved in the bruising. Now as I mentioned previously that in your case it can be normal phenomenon or can be due to certain other problems which you have mentioned in your medical history.

At certain times there are certain medications which also can cause interference with normal coagulation mechanism. Pain killer medications can cause interference in the coagulation pathway. So you should tell your doctor about the present episode of purpuric spots.

For the best part of the diagnosis and rule out any abnormality I suggest my patients for simple tests of platelet function and coagulation factors. You can wait for few hours and if the spots are increasing then can meet your primary care physcain to get tested for your coagulation profile.

These are done routinely in all the simple laboratories to rule out the exact cause of the problem. If these tests are normal then you can rest assured and relieve your tensions regarding the rash.

I suggested you to wait and watch for the lesions only in case if these are not spreading. In case if these are spreading or becoming larger in size then you have to immediately meet your primary care physician to rule out the exact cause.

You let me know whether these are increasing in size and number or are become more darker as compared to morning.

They are not increasing in size are not getting darker and are not increasing in number they seem to have stopped spreading

Hello young lady,

Welcome again to the forum.

It is a point of relief that the spots are not increasing in size or number as well as there is no any other symptom related to it.

In this latest scenario I would like to keep my diagnosis of purpura due to normal physiological cause. Many a time we see in our clinical practice that within few days after menses one can get purpuric spots. This is due to the change in hormonal levels well as role of certain chemicals (fibrinogen).

So you can very well wait and watch for the time being period. In my view this should not increase now and therefore can relax.

But I would also like to suggest you to keep a watch and inform your treating doctor regarding this episode for future reference especially in case of pain killers.
