Question: I had a small spot of numbness at the back of my heel 18 months ago, which gradually spread forward on the side of my heel. Recently I have been awakened by intense burning sensation in my toes, especially 3, 4, 5 and when I first put my foot on the floor the ball of my foot feels lumpy. My doctor has put me on Lyrica which is on trolling the night pain. He suspected Mortons Neuroma, but ultrasound and X-ray didn't confirm.

Hello sir,
I have read your case history and i think you are having pinch nerve in back. This nerve is compressed due to disc herniation. Its compression is causing altered sensation and burning in foot and heel.
Other possibility can be vitamin B 12 deficiency.
I do not think that it can be Morton's neuroma as pain in that situation is main symptoms and usually it occurs at tip of the toes.

I would start you on pregabalin and methycobal for a month and see if you feel better. Your local doctors will know about it. If your symptoms persist or increase in one month then you can get serum B 12 estimation and MRI of the spine and foot. This will confirm the diagnosis.

I have been taking a multi B vitamin with 50 mcg of B12. Is that enough? Since I have a pacemaker, an MRI is not possible. Would X-ray's be sufficient? I saw a neurosurgeon two years ago and he said my L4 L5 was compressed but not enough to warrant treatment. Any thoughts?

Hello,
If MRI is not be possible in your case. I would suggest you to get CT myelogram. In this test they use XXXXXXX and check for disc herniation. X ray is not helpful for this problem.
It is good that you are already taking multivitamin. But doses of B 12 is not enough. If you would be my patient then I would give you Pregabalin M tablet.
Since 2 year has passed when you visited neurosurgeon, your disc compression may have increased. It is better to get either CT Myelogram or EMG/NCV to check if disc compression is cause of your symptoms.
