default
Posted on Tue, 30 Jun 2015
Question: My pulse rate while laying in bed has been anywhere between 48 and 57 beats a minute is this ok I am 58 yrs old and have about 6 to 7 irregular beats per minute sometimes less. I am seeing my cardiologist next week but when I was there for a stress test a year ago he said it was not abnormal to have irregular beats at my age.
He discovered a couple of years ago that I have about 50 percent blockage in my right artery what is your take.

Thanks,
XXXXXXX Ps have not shortness of breath or chest pains

Detailed Answer:
Hello XXXX!

Thank you for asking on HCM!

Regarding your concern I would explain that when lying down or sleeping, heart rate is lower than when on physical activity or awaken. Your 48 to 57 bpm is quite normal. So don't worry!

A 50% blockage of RCA is considered clinically non significant. As long as you don't suffer from chest pain or dyspnea, the best thing to do is closely monitoring eventual symptomatology.

Ectopic heart beats, 5 to 7 beats per minute are not considered normal, and facing that you have a prior 50% stenosis of your RCA, reveals need for paying special attention on the issue, as arrhythmic events are sometimes the innitial and the only prodromal of a serious ischemic heart event.

At that point, I would recommend to perform an ambulatory ECG monitoring (Holter), and a cardiac stress test to rule in/out a possible ischemic heart disorder in the near future, and to define the need for undergoing a coronary angiography.

Hope to have been helpful to you!

Feel free to ask me whenever you need! Greetings! Dr. Iliri
