What Causes Slight Vaginal Bleeding Post Menopause?

Question: Hi Doctor:

I had scant vaginal bleeding x1. I am postmenopausal, 60 years old. I went to the doctor. She sent me for pelvic ultrasound and transvaginal ultrasound. Results indicated as follows uterus is heterogeous in echotexture. There are fibroids. There is a submucosal isoechoic mass which lies posterior to endometrium. Partially calcified exohytic nodule projects from the posterior margin of the fundus.
There is nonspecific fluid within the endometrial cavity. Endometrial thickness =4.4 mm. Ovaries are intact. No focal ovarian or adnexal abnormalities. No free fluid in cul-de-sac. Nonspecific trace fluid in endometrial cavity no evidence of abnormal endometrial thickening. Doctor called me after three weeks to tell me she wants to do a biopsy of the endometrial fluid. When I didn't hear from her regarding the results for so long, I thought everything was fine. Could this be cancer?

I understand your concern..

Yes, what your doctor did was right.. Since the scan report says you have some fluid in the endometrial cavity, the fluid needs to be sampled along with a pap smear to confirm that you dont have any cervical lesions.. The report of the test is important to reach to a diagnosis.. With the reports the likelyhood of cancer can be ruled out..

Most common cause of postmenopausal bleeding is atrophic vaginitis which usually subsides on local application of estrogen cream.. If all the reports are normal, then it is likely to be atrophic vaginitis..

Fibroids is unlikely to cause postmenopausal bleeding as it shrinks after menopause..

Hi again Dr. Dalia:

Thank you so much for a quick reply to my inquiries. My concern is that from the last transvaginal us one fibroid has grown just slightly also there is an extrinsic mass effect on the endometrium could this also be a fibroid? Also not sure what nonspecific trace fluid in the endometrial cavity is. What does nonspecific trace fluid mean. Radiologist not sure what the fluid is they called it indeterminate. What kind of fluid could accumulate there. If this is cancer, would it be at an early state being that the endometrial thickening is =4.4. Again, thank you for making things clear.

Also doctor would like to mention to you that I had a pap smear; pap sure just three months ago everything is fine.

Thanks for writing..
Endometrial fluid need to be evaluated and if normal nothing needs to be done.. Follow up every 6 monthly to see if there is sudden growth of fibroids.. After menopause fibroid is unlikely to cause bleeding..

Get a hysteroscopy to see the lesion which is causing pressure effect on endometrium..

Since pap smear is normal, it need not be repeated..
