Posted on Fri, 24 Oct 2014
Question: The past few weeks I have had shooting pains in my lower spine when i am sitting and worse at night when I am in bed. It continues to hurt worse and worse. I do have scoliosis so I am used to back pain but this is a different pain. I have tried ibphophen and naproxen both prescription strength and they don't make a difference. I have also tried heat. Any suggestions or should I go see my doctor?

Brief Answer:
May be disc prolapse.

Detailed Answer:
Hi,
Thanks for your query.
I'm Dr. Sudhir Kumar,

After going through your query I came to know that you may be suffering from lumbar disc prolapse as it is common in your age group and pain is shooting type and aggravated by sitting.
It can be confirmed by MRI scan.

Its primary treatment is bed rest in position of relief(generally lateral position with both hip and knee bend).

Neurotopics such as mecobalamin and analgesics are helpful. You has already tried ibuprofen and naproxen. Sometimes strong analgesic(such as tramadol three times a day after meals) is required. Tramadol has tendency to cause nausea and vomiting so it is advisable to take anti emetic along with it. You can discuss with your treating Doctor about it.
I want to know two clarification from you.
1. Whether pain started after some weight lifting or pushing heavy objects or other strenuous work?
2. Whether pain is relieved by lying down and aggravated by walking?

Second possibility is muscular pain. Treatment of it is rest and analgesics for nearly 5 days.

Hope I have answered your query.
If you have any further questions I will be happy to help.
Wishing you good health.
Take care.

It was a few weeks ago so I am not sure what i would have been doing. It is not relieved by laying down that is one of the worst times and walking seems to make it feel a little better.

Brief Answer:
Disc prolapse first possibility.

Detailed Answer:
Hi,

Welcome back,

After going through your query and follow up query I think first possibility is of disc prolapse looking to your age and shooting pain. But pain is not relieved by laying down is against it. I think in disc prolapse relatively strict bed rest is advised. If pain does not reduce in intensity even after strict bed rest in previously discussed position along with other advise then you need to investigate yourself looking for other causes by MRI spine.Vitamin B-12 test and vitamin D test are also advisable and you are advised to consult your doctor. Hope I have answered your query.

If you have any further questions I will be happy to help.

If you do not have any clarifications, you can close the discussion and rate the answer.

Wishing you good health.
Take care.
